摘  要:目的了解新乡医学院第三附属医院临床各科室送检血培养病原菌检出及药敏情况,指导临床合理用药。方法对新乡医学院第三附属医院2019年1月1日至2020年12月31日临床送检的血培养标本检测结果进行回顾性统计分析。结果全院共送血培养5348套,位居前5名的科室分别是儿科、呼吸内科、重症医学科、神经外科、新生儿科;共报阳408套,阳性率7.6%,检出革兰阳性球菌181套(44.4%),阴性杆菌222套(54.4%),真菌5套(1.2%)。检出病原菌前5位分别是大肠埃希菌,凝固酶阴性葡萄球菌,肺炎克雷伯菌,金黄色葡萄球菌,鲍曼不动杆菌。大肠埃希菌、肺炎克雷伯菌、鲍曼不动杆菌、铜绿假单胞菌对碳青霉烯类耐药率分别是0、7.0%、42.9%、15.4%。未发现对万古霉素、利奈唑胺耐药的葡萄球菌及肠球菌。结论血流感染病原菌种类繁多,不同病原菌耐药性差异很大,检验科微生物室应加强细菌耐药监测,为临床合理用药提供依据。Objective To investigate the detection and drug sensitivity of pathogenic bacteria in blood culture submitted by all clinical departments in the Third Affiliated Hospital of Xinxiang Medical University and provide reference to use antibiotics rationally for clinical.Methods A retrospective statistical analysis was performed on the results of blood culture samples submitted in the Third Affiliated Hospital of Xinxiang Medical University from January 1,2019 to December 31,2020.Results A total of 5348 sets of blood culture were delivered,and the top five departments were Pediatrics,Respiratory Medicine,ICU,Neurosurgery and Neonatology.A total of 408 positive cases were reported,with a positive rate of 7.6%.A total of 181 cases of Gram-positive bacterium(44.4%),222 cases of Gram-negative bacilli(54.4%)and 5 cases of fungi(1.2%)were detected.The top five pathogens detected were Escherichia coli,coagulase-negative Staphylococcus,Klebsiella pneumoniae,Staphylococcus aureus and Acinetobacter baumannii.The drug resistance rates of Escherichia coli,Klebsiella pneumoniae,Acinetobacter baumannii and Pseudomonas aeruginosa to carbapenems were 0,7.0%,42.9%and 15.4%,respectively.There were no Staphylococcus and Enterococcus resistant to vancomycin and linezolid were found.Conclusion There are many kinds of pathogenic bacteria in bloodstream infection,and the drug resistance of different pathogens varies greatly.Microbiology laboratory of clinical laboratory should strengthen the monitoring of bacterial drug resistance,so as to provide the basis for the drug using rationally.
