纤维支气管镜下支气管结核冷冻对肉芽增殖型支气管结核的疗效与安全性研究

摘　　要：目的观察肉芽增殖型支气管结核者行纤维支气管镜下支气管结核冷冻治疗的效果。方法选取2019年8月-2020年8月肉芽增殖型支气管结核者62例,行普通抗结核治疗的31例是参考组,增加纤维支气管镜下支气管结核冷冻治疗的31例是治疗组,比较两组的效果。结果治疗组治疗总有效率93.55%,比参考组(70.97%)更优,差异有统计学意义(χ^(2)=5.415,P<0.05)。治疗组并发症发生率是9.68%,参考组35.48%,前者比后者更安全,差异有统计学意义(χ^(2)=5.905,P=0.015)。比较LDH、ADA两项指标,治疗组和参考组在治疗前差异无统计学意义(t=0.133、0.327,P>0.05)。治疗后治疗组LDH、ADA水平更低,差异有统计学意义(t=12.148、10.499,P<0.05)。结论在肉芽增殖型支气管结核者的治疗中,纤维支气管镜下支气管结核冷冻的疗效好,安全性高。Objective To observe the effect of bronchial tuberculosis cryotherapy under fiberoptic bronchoscopy in patients with granulomatous bronchial tuberculosis.Methods From August 2019 to August 2020,62 cases of granulomatous bronchial tuberculosis were selected,31 cases of ordinary anti-tuberculosis treatment were the reference group,31 cases of bronchial tuberculosis under fibrobronchoscope cryotherapy were the treatment group,and the effects of the two groups were compared.Results The total effective rate of treatment in the treatment group 93.55%was better than the reference group(70.97%),and the difference was statistically significant(χ^(2)=5.415,P<0.05).The complication rate in the treatment group was 9.68%and 35.48%in the reference group.The former was safer than the latter,and the difference was statistically significant(χ^(2)=5.905,P=0.015).Comparing the two indicators of LDH and ADA,there was no statistically significant difference between the treatment group and the reference group before treatment(t=0.133,0.327,P>0.05).After treatment,the levels of LDH and ADA in the treatment group were lower,and the difference was statistically significant(t=12.148,10.499,P<0.05).Conclusion In the treatment of granulomatous bronchial tuberculosis patients,bronchial tuberculosis freezing under fiberoptic bronchoscopy has good curative effect and high safety.
