后外侧联合前内侧入路治疗复杂Pilon骨折的效果及安全性分析

摘　　要：目的探究后外侧联合前内侧入路治疗复杂Pilon骨折的效果及安全性。方法方便选取2018年1月—2020年3月该院收治的复杂Pilon骨折患者70例为观察对象行对比调查,参考治疗方案的不同,将前内侧入路治疗患者纳入到A组,将后外侧联合前内侧入路治疗患者纳入B组,观察不同方案应用效果。结果A组患者复位优良率为77.14%,小于B组的97.14%,组间患者复位效果对比,差异有统计学意义(χ^(2)=4.590,P<0.05)。A组患者手术时间与术后住院时间及骨折愈合时间均长于B组,差异有统计学意义(t=5.519、2.394、3.690,P<0.05);A组中并发症发生率为5.71%,B组并发症发生率为8.57%,组间并发症发生率对比,差异无统计学意义(χ^(2)=0.000,P>0.05)。结论针对复杂Pilon骨折的治疗,采用后外侧联合前内侧入路治疗患者术后复位效果良好,且有助于缩短患者术后康复时间,同时还有助于改善骨代谢情况。Objective To explore the effect and safety of the posterolateral combined anteromedial approach in the treatment of complex Pilon fractures.Methods From January 2018 to March 2020,convenience selection 70 patients with complex Pilon fractures admitted to the hospital as observation objects for comparative investigation.With reference to the different treatment plans,the patients treated with the anteromedial approach were included in group A,and the posterolateral combination was combined.Patients treated with anteromedial approach were included in group B,and the effects of different programs were observed.Results The excellent and good rate of reduction in group A was 77.14%,which was less than 97.14%in group B.There was a statistically significant difference in the reduction effect between groups(χ^(2)=4.590,P<0.05).The operation time,postoperative hospital stay and fracture healing time of group A were longer than those of group B,and the difference was statistically significant(t=5.519,2.394,3.690,P<0.05);the incidence of complications in group A was 5.71%;group B complication incidence rate was 8.57%,and there was no statistically significant difference in the incidence of complications between the groups(χ^(2)=0.000,P>0.05).Conclusion For the treatment of complex Pilon fractures,the posterolateral combined anteromedial approach has a good postoperative reduction effect,and helps to shorten the postoperative recovery time of the patient,and also helps to improve the bone metabolism.
