加味复元活血汤联合椎体成形术对老年骨质疏松性胸腰椎压缩骨折患者炎症指标及疼痛程度的影响

摘　　要：目的探讨加味复元活血汤联合椎体成形术对老年骨质疏松性胸腰椎压缩骨折患者炎症指标及疼痛程度的影响。方法方便选取2017年6月—2019年12月该院90例老年骨质疏松性胸腰椎压缩骨折患者,按随机数表法分为对照组(n=45)和观察组(n=45)。对照组给予椎体成形术治疗,观察组则于对照组基础上联合加味复元活血汤治疗。比较两组治疗前、治疗2周时血清炎症指标[肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)]及疼痛程度[视觉模拟评分法(VAS)]、功能状况[奥斯维斯特功能障碍指数问卷表(ODI)]。结果治疗2周,观察组血清TNF-α、hs-CRP、PCT水平分别为(337.14±32.11)μg/1、(8.23±0.92)mg/L、(3.71±0.25)μg/L,均低于对照组的(420.78±41.82)μg/L、(11.23±1.10)mg/L、(6.95±0.63)μg/L,差异有统计学意义(t=10.641、14.034、32.087,P<0.001);治疗2周,观察组VAS评分为(1.28±0.46)分,低于对照组(3.36±0.57)分,差异有统计学意义(t=19.050,P<0.001);治疗2周,观察组ODI评分为(23.41±1.98)分,低于对照组(29.62±2.07)分,差异有统计学意义(t=14.543,P<0.001)。结论加味复元活血汤联合椎体成形术可有效降低老年骨质疏松性胸腰椎压缩骨折患者炎症指标水平,降低疼痛程度,改善功能状况。Objective To investigate the effect of Jiawei Fuyuan Huoxue Decoction combined with vertebroplasty on inflammation indexes and pain degree of elderly patients with osteoporotic thoracolumbar compression fractures.Methods A total of 90 elderly patients with osteoporotic thoracolumbar compression fractures in the hospital from June 2017 to December 2019 were convenicence selected and divided into a control group(n=45)and an observation group(n=45)according to a random number table.The control group was treated with vertebroplasty,and the observation group was treated with Jiawei Fuyuan Huoxue Decoction on the basis of the control group.Comparison of serum inflammatory indicators[tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT)]and pain degree[visual simulation scoring method(VAS)],functional status[oswestern dysfunction index questionnaire(ODI)]before treatment and 2 weeks after treatment between the two groups.Results After 2 weeks of treatment,the serum TNF-α,hs-CRP and PCT levels in the observation group were(337.14±32.11)μg/L,(8.23±0.92)mg/L,and(3.71±0.25)μg/L,all lower than the control group(420.78±41.82)μg/L,(11.23±1.10)mg/L,(6.95±0.63)μg/L,the difference were statistically significant(t=10.641,14.034,32.087,P<0.001);after 2 weeks of treatment,the VAS score of the observation group was(1.28±0.46)points,which was lower than the control group(3.36±0.57)points,the difference was statistically significant(t=19.050,P<0.001);after 2 weeks of treatment,the ODI score of the observation group was(23.41±1.98)points,which was lower than the control group(29.62±2.07)points,the difference was statistically significant(t=14.543,P<0.001).Conclusion Modified Fuyuan Huoxue Decoction combined with vertebroplasty can effectively reduce the level of inflammatory indicators in elderly patients with osteoporotic thoracolumbar compression fractures,reduce pain and improve functional status.
