热带地区螨虫相关性眼病的治疗方法探讨

摘　　要：目的探讨热带地区螨虫相关性眼病的治疗方法,旨在寻找一种有效清除睑缘蠕形螨,减轻睑缘炎症,提升眼表舒适度的治疗方式。方法采用重复测量设计方法,选择2018年9月—2019年3月在海南省人民医院门诊就诊的螨虫相关性睑缘炎患者56例(112只眼),随机分为试验组(28例56只眼)、对照组(28例56只眼)。试验组行热敷、睑板腺按摩后采用茶树精油进行睑缘清洁,对照组行热敷、睑板腺按摩后采用生理盐水进行睑缘清洁,两组患者均滴用玻璃酸钠滴眼液,分别对治疗前、治疗2周、治疗1个月、治疗2个月时的OSDI评分、睑缘评分、BUT进行评估,同时评估两组治疗前、治疗2个月后的螨虫镜检阳性率。结果治疗前两组四项指标差异均无统计学意义(P>0.05),治疗2周时两组OSDI评分差异无统计学意义(P>0.05),试验组睑缘评分比对照组低,BUT比对照组延长;治疗1个月、2个月时两组对比,试验组OSDI评分、睑缘评分比对照组低,BUT比对照组延长。治疗2周时,试验组OSDI评分较入组时差异无统计学意义(P>0.05),睑缘评分比入组时低,BUT比入组时延长;治疗1个月、2个月时,试验组OSDI评分、睑缘评分均比入组时低,而BUT比入组时延长;对照组在治疗后三个研究时间段,睑缘评分均比入组时低,OSDI评分、BUT这两个指标在各研究时间段与入组时相较差异无统计学意义(P>0.05)。治疗2个月后试验组患者螨虫镜检阳性率为21.4%,对照组为89.3%,两组比较差异有统计学意义(P<0.05)。结论睑板腺热敷、按摩后采用茶树精油清洁睑缘联合玻璃酸钠滴眼液能有效缓解螨虫相关性睑缘炎的干眼症状、睑缘炎症,有效清除睑缘蠕形螨,促进泪膜功能恢复,提升眼部舒适度,尤为适用于热带地区的螨虫相关性眼病患者。Objective To explore the treatment of mite associated ophthalmopathy in tropical areas,the objective is to find an effective treatment method to remove demodex blepharoides,reduce blepharoides inflammation and improve the comfort of ocular surface.Methods A prospective study was conducted.56 patients(112 eyes)with mite associated blepharitis who visited the outpatient department of Hainan Provincial People's Hospital from September 2018 to March 2019 were randomly divided into experimental group(28 cases,56 eyes)and control group(28 cases,56 eyes).After hot compress and meibomian gland massage,tea tree oil was used to clean the eyelid margin in the experimental group,and normal saline was used to clean the eyelid margin in the control group.Sodium hyaluronate eye drops were used in all patients.The OSDI score,eyelid margin score and tear film rupture time(BUT)were evaluated at the beginning of treatment,2 weeks,1 month and 2 months respectively,meanwhile the positive rates of mite microscopic examination before treatment and 2 months after treatment were evaluated.Results There was no significant difference in four indexes between the two groups at the beginning of treatment(P>0.05),at 2 weeks there was no significant difference in OSDI score between the two groups(P>0.05),the eyelid margin score of the experimental group was lower than that of the control group,BUT was longer than that of the control group;at 1 month and 2 months,the OSDI score and the eyelid margin score of the experimental group were lower than that of the control group,BUT was longer than that of the control group.At 2 weeks,there was no significant difference in OSDI score(P>0.05)compared with that before treatment,but lower eyelid margin score and longer BUT compared with that before treatment in the experimental group;at 1 month and 2 months,OSDI score and eyelid margin score were lower and BUT was longer compared with that before treatment in the experimental group;in the three study periods after treatment,the eyelid margin scores were l
