补正续骨丸联合自我效能增强干预对老年全膝关节置换术后患者康复及生活质量的影响

摘　　要：目的探讨补正续骨丸联合自我效能增强干预对老年全膝关节置换术后患者康复及生活质量的影响。方法收集2019年4月-2020年1月我院诊治的老年全膝关节置换术后患者88例,随机分为2组,各44例,对照组予以术后常规康复治疗及护理,观察组在此基础上予补正续骨丸联合自我效能增强干预,疗程30 d,随访6个月。结果两组术后膝关节功能评分(HSS)均呈升高趋势,且组间比较,术后1个月两组差异无统计学意义(P>0.05),术后3、6个月,观察组较对照组升高更显著(P<0.05);两组术后自我效能感评分(SER)均逐渐升高,且组间比较,术后1个月两组SER评分差异无统计学意义(P>0.05),术后3、6个月,观察组较对照组升高更显著(P<0.05);术后3、6个月,两组生活质量评分(SF-36)较前有明显上升(P<0.05),且组间比较,观察组较对照组升高更显著(P<0.05);观察组的临床总有效率优于对照组(88.64%vs.70.45%,P<0.05)。结论补正续骨丸具有补益正气、补肾强骨、活血止痛之效,与自我效能增强干预联合应用后,在提高生活质量、增强自我效能意识和恢复膝关节功能方面均有显著疗效,可改善老年全膝关节置换术后患者临床症状,临床疗效较好。Objective To explore the effect of Buzhengxugu bolus combined with self-efficacy enhancement intervention on rehabilitation and life quality of elderly patients with total knee arthroplasty.Methods The information of 88 patients with total knee arthroplasty diagnosed and treated in our hospital from April 2019 to January 2020 was collected and were divided into two groups in a random way,of which 44 patients in the control group(group A)were treated with routine rehabilitation treatment and nursing(Scheme A)and 44 patients in the treatment group(group B)were treated with Buzhengxugu bolus combined with self-efficacy enhancement intervention(Scheme B)besides Scheme A.The treatmen course was 30 days.The patients were followed up for 6 months.Results The HSS scores of both groups showed an increasing trend after operation,and there was no significant difference between the two groups one month after operation(P>0.05).At 3 and 6 months after operation,the score of the observation group increased more significantly than that of the control group(P<0.05).The postoperative SER scores of the two groups increased gradually,and there was no significant difference between the two groups one month after operation(P>0.05).At 3 and 6 months after operation,the score of the observation group increased more significantly than that of the control group(P<0.05).At 3 and 6 months after operation,the scores of SF-36 in the two groups were significantly higher than baseline(P<0.05),and the scores in the observation group were significantly higher than those in the control group(P<0.05).The clinical total effective rate of the observation group was higher than that of the control group(88.64%vs.70.45%,P<0.05).Conclusion Buzhengxugu bolus has the effects of enriching the primordial qi,tonifying kidney and strengthening bone,activating blood circulation and relieving pain.Combined with self-efficacy intervention,it can significantly improve the recovery of knee joint function,enhance self-efficacy awareness,improve the quality of life a
