充气式保温毯联合输液加温在老年全髋关节置换术后加速康复中的应用

摘　　要：目的:探讨充气式保温毯联合输液加温在老年全髋关节置换患者术后加速康复中的应用价值。方法:选取广西壮族自治区人民医院2018年6月至2019年12月行单侧全髋关节置换术的老年患者90例,随机分为对照组(n=45)和干预组(n=45)。对照组术后采用常规保温方法,干预组在常规保温措施基础上增加充气式保温毯联合输液加温。比较两组复温幅度与速度、术后并发症发生率、术后伤口引流量、拔管时间、首次下床活动时间、住院日、出院时Harris评分等指标。结果:干预组术后复温幅度与速度显著优于对照组,差异均有统计学意义(均P<0.05);两组术后并发症发生率、术后伤口引流量、拔管时间、首次下床活动时间、住院日、出院时Harris评分差异均有统计学意义(均P<0.05)。结论:采用充气式保温毯联合输液加温可以快速提高老年全髋关节置换术患者复温幅度,降低术后并发症的发生率,有利于患者术后加速康复。Objective:To study the application of inflatable insulation blanket combined with infusion heating in elderly patients after total hip arthroplasty.Methods:The ninety elderly patients with unilateral total hip arthroplasty in People's Hospital of Guangxi Zhuang Autonomous Region from June 2018 to December 2019 were randomly divided into control group and experimental group,45 cases in each group.The control group was treated with conventional insulation method,while the experimental group was treated with inflatable insulation blanket combined with infusion heating on the basis of conventional insulation measures.The range and speed of rewarming,incidence of postoperative complications,postoperative wound drainage,extubation time,first ambulation time,length of stay and Harris score at discharge were compared between the two groups.Results:The amplitude and speed of rewarming in the experimental group were significantly better than those in the control group,and the difference between the two groups was statistically significant(all P<0.05);the incidence of postoperative complications,postoperative wound drainage,extubation time,first ambulation time,length of stay and Harris score at discharge between the two groups were statistically significant(all P<0.05).Conclusions:The use of inflatable insulation blanket combined with infusion heating can quickly improve the rewarming range of elderly patients with hip replacement,effectively reduce the incidence of postoperative complications,which is conducive to the rapid recovery of patients.
