摘　　要：目的分析前交叉韧带胫骨止点撕脱骨折患者应用关节镜下不可吸收缝线和金属带线锚钉固定的临床效果。方法80例前交叉韧带胫骨止点撕脱骨折患者,按照随机数字抽样方式分为观察组和对照组,各40例。观察组采用关节镜下不可吸收缝线固定,对照组采用关节镜下金属带线锚钉固定。比较两组手术指标、术后关节功能、并发症发生率。结果观察组患者手术时间(1.19±0.85)h、骨愈合时间(3.24±0.43)个月短于对照组的(1.80±0.93)h、(3.96±0.65)个月,手术费用(1.37±0.62)万元低于对照组的(2.08±0.76)万元,差异有统计学意义(P<0.05)。术后6个月,观察组患者膝关节活动度、Tegner评分、Lysholm评分、IKDC评分均高于对照组,差异有统计学意义(P<0.05)。观察组患者并发症发生率为5.0%,与对照组的7.5%比较,差异无统计学意义(P>0.05)。结论关节镜下不可吸收缝线和金属带线锚钉两种固定治疗均有一定的治疗效果,而不可吸收缝线固定方式具有操作便捷、治疗费用低、康复效果明显的优点,值得临床研究和推广应用。Objective To analyze the clinical effect of non-absorbable suture fixation and metal suture anchor fixation in patients with avulsion fracture of tibial insertion of anterior cruciate ligament under arthroscopy.Methods A total of 80 patients with avulsion fracture of tibial insertion of anterior cruciate ligament were divided into the observation group and the control group according to random numerical table,with 40 cases in each group.The observation group was fixed with non-absorbable suture under arthroscopy,and the control group was fixed with metal suture anchor under arthroscopy.The surgical index,postoperative joint function,and complication rate were compared between the two groups.Results The operation time(1.19±0.85)h and bone healing time(3.24±0.43)months in the observation group were shorter than(1.80±0.93)h and(3.96±0.65)months in the control group,and the operation cost(1.37±0.62)million yuan was lower than(2.08±0.76)million yuan of the control group,and the differences were statistically significant(P<0.05).6 months after operation,the knee range of motion,Tegner score,Lysholm score,and IKDC score of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).The complication rate of patients in the observation group was 5.0%,which was not statistically significant compared with 7.5%in the control group(P>0.05).Conclusion Both non-absorbable suture and metal suture anchors fixation under arthroscopy have certain therapeutic effects.The non-absorbable suture fixation has the advantages of convenient operation,low cost,and obvious rehabilitation effect,which is worthy of clinical research and popularization.
