糖皮质激素治疗老年重症社区获得性肺炎的效果及对患者血清炎症因子水平的影响

摘　　要：目的探究糖皮质激素治疗老年重症社区获得性肺炎的效果及对患者血清炎症因子水平的影响。方法245例老年重症社区获得性肺炎患者为研究对象,随机分为对照组(122例)和观察组(123例)。对照组采用常规治疗,观察组在对照组基础上联合糖皮质激素治疗。比较两组患者住院时间、再感染率、治疗前后炎性因子水平、治疗效果。结果治疗后,观察组患者住院时间(15.24±4.75)d短于对照组的(18.33±5.72)d,再感染率2.44%低于对照组的12.30%,差异具有统计学意义(P<0.05)。观察组患者IL-8(70.69±8.01)pg/ml、IL-6(24.46±2.65)pg/ml、TNF-α(67.36±6.88)pg/ml均显著低于对照组的(91.22±10.04)、(38.62±3.96)、(79.27±8.07)pg/ml,治疗总有效率98.36%高于对照组的82.93%,差异具有统计学意义(P<0.05)。结论糖皮质激素可显著缩短老年重症社区获得性肺炎患者机体恢复时间,改善血清炎症因子水平,临床应用效果显著。Objective To investigate the effect of glucocorticoid in the treatment of elderly patients with severe community-acquired pneumonia and its influence on serum inflammatory factors.Methods A total of 245 elderly patients with severe community-acquired pneumonia were studied and randomly divided into control group(122 cases)and observational group(123 cases).The control group was treated with conventional therapy,and the observational group was treated with glucocorticoid based upon the control group.Patients in both groups were compared in terms of hospitalization time,reinfection rate,inflammatory factor levels before and after treatment,and therapeutic effect.Results After treatment,the hospitalization time(15.24±4.75)d in the observation group was shorter than(18.33±5.72)d in the control group,and the reinfection rate 2.44%was lower than 12.30%in the control group.All the differences were statistically significant(P<0.05).IL-8(70.69±8.01)pg/ml,IL-6(24.46±2.65)pg/ml and TNF-α(67.36±6.88)pg/ml in the observation group were significantly lower than(91.22±10.04),(38.62±3.96)and(79.27±8.07)pg/ml in the control group,and the total effective rate of treatment 98.36%was higher than 82.93%in the control group.All the differences were statistically significant(P<0.05).Conclusion Glucocorticoid has significant clinical effect on elderly patients with severe community-acquired pneumonia,and it can significantly shorten the recovery time of patients,and improve the level of serum inflammatory factors.
