摘 要:目的分析预混胰岛素皮下注射与胰岛素泵治疗糖尿病的效果和不良反应发生情况。方法70例糖尿病患者,随机分为对照组和观察组,每组35例。对照组患者给予预混胰岛素常规皮下注射,观察组患者给予胰岛素泵治疗。比较两组血糖恢复正常时间、住院时间、胰岛素用量、治疗前后血糖水平、整体治疗效果、不良反应发生情况。结果观察组患者血糖恢复正常时间(7.21±1.23)d、住院时间(12.21±1.25)d均短于对照组的(12.56±2.91)、(15.56±2.21)d,胰岛素用量(38.19±2.51)U/d低于对照组的(49.25±3.18)U/d,差异均具有统计学意义(P<0.05)。治疗后,观察组餐后2 h血糖、空腹血糖改善程度大于对照组,差异具有统计学意义(P<0.05)。观察组整体治疗总有效率94.29%高于对照组的74.29%,差异具有统计学意义(χ^(2)=5.285,P<0.05)。观察组不良反应发生率2.86%低于对照组的20.00%,差异具有统计学意义(χ^(2)=5.081,P<0.05)。结论胰岛素泵治疗糖尿病的效果优于预混胰岛素皮下注射的患者,且用药的安全性明显提高,值得推广和应用。Objective To analyze the effect of subcutaneous injection of premixed insulin versus insulin pump in the treatment of diabetes mellitus and the adverse reactions of patients.Methods A total of 70 patients with diabetes mellitus were randomly divided into control group and observation group,with 35 cases in each group.Patients in the control group was treated with subcutaneous injection of premixed insulin,and patients in the observation group was treated with insulin pump.Both groups were compared in terms of time to return to normal blood glucose,hospitalization time,insulin dosage,blood glucose level before and after treatment,overall treatment effect,and occurrence of adverse reactions.Results The time to return to normal blood glucose(7.21±1.23)d and hospitalization time(12.21±1.25)d in the observation group were shorter than(12.56±2.91)and(15.56±2.21)d in the control group,and the insulin dosage(38.19±2.51)U/d was lower than(49.25±3.18)U/d in the control group;all the differences were statistically significant(P<0.05).After treatment,the improvement of 2 h postprandial glucose,and fasting blood glucose level in the observation group was greater than those in the control group,and the difference was statistically significant(P<0.05).The total effective rate of overall treatment 94.29%in the observation group was higher than 74.29%in the control group,and the differences were all statistically significant(χ^(2)=5.285,P<0.05).The incidence of adverse reactions 2.86%in the observation group was lower than 20.00%in the control group,and the difference was statistically significant(χ^(2)=5.081,P<0.05).Conclusion Insulin pump has better effect for the treatment of diabetes mellitus than those of subcutaneous injection of premixed insulin,and the safety of medication is significantly improved,which is worthy of promotion and application.
