河南省中医类医院卫生资源配置公平性评价

摘　　要：目的评价河南省中医类医院卫生资源配置公平性,为优化河南省中医药卫生资源提供参考依据。方法利用基尼系数和泰尔指数,从人口维度评价2014—2018年河南省中医类医院卫生资源配置公平性。结果与2014年相比,2018年河南省中医类医院卫生技术人员、实有床位和财政补助收入分别增加了36.97%、40.37%和179.51%;每千人口卫生技术人员、实有床位与全国同期水平持平,每千人口财政补助收入低于全国同期平均水平。基尼系数显示,卫生技术人员、实有床位和财政补助收入按人口配置的基尼系数分别在0.2136~0.2257、0.2040~0.2295和0.3379~0.4345之间,均呈下降趋势;泰尔指数结果显示,河南省中医类医院卫生技术人员、实有床位和财政补助收入的总体泰尔指数分别在0.0754~0.8833、0.0669~0.0834和0.1252~0.3238之间,各项指标区域内泰尔指数及差异贡献率均远高于区域间泰尔指数及差异贡献率。结论河南省中医类医院各项卫生资源配置量逐渐增加,卫生资源配置公平性提升,公平性差异主要来源于区域内部差异,财政补助收入资源分布均衡性较差,需进一步落实财政倾斜政策。Objective To analyze the status and equity of health resource allocation in Traditional Chinese Medicine(TCM) hospitals in Henan province, and to provide evidence for optimizing TCM health resources in Henan province. Methods Gini coefficient and Theil index were used to evaluate the allocation of health resources in Traditional Chinese Medicine hospitals in Henan province from 2014 to 2018. Results Compared with 2014, the income of health technicians, actual beds and financial subsidy income in TCM hospitals in Henan province in 2018 increased by 36.97%, 40.37% and 179.51%, respectively. The number of medical technicians and actual beds per 1 000 population was the same as the national average in the same period, and the financial subsidy income per 1 000 population was lower than the national average in the same period. Gini coefficient showed that the Gini coefficient of health technicians, actual beds and financial subsidy income according to population was 0.213 6-0.225 7, 0.204 0-0.229 5 and 0.337 9-0.434 5, respectively, showing a downward trend. Theil index results showed that the overall Theil index of health technicians, actual beds and financial subsidy income in TCM hospitals in Henan province was 0.075 4-0.883 3, 0.066 9-0.083 4 and 0.125 2-0.323 8, respectively. Theil index and variance contribution rate of each index within the regions were both much higher than that between the regions. Conclusion The allocation of various health resources in TCM hospitals in Henan province is gradually increasing, and the fairness of health resources allocation is improved. The equity difference mainly comes from the intra-regional differences. The distribution balance of financial subsidy income and resources is poor, and the financial tilt policy needs to be further implemented.
