摘 要:目的研究核素肾动态显像法用于肾积水患者肾功能评估的临床价值。方法选取2019年1月—2020年5月新乡市中心医院接收的肾积水患者96例,采用肾穿刺造瘘术确认患者有无功能,有功能者50例作为A组,肾无功能者46例作为B组,再采用彩色多普勒超声诊断仪对两组患者的RI进行测定,采用核素肾动态显像法对患者的肾小球滤过率(GFR)进行测定,对比两组的肾内血流阻力指数(RI)、GFR检测结果。结果 B组患者的RI明显高于A组,差异有统计学意义(P<0.05);B组患者的GFR低于A组,差异有统计学意义(P<0.05);两组患者的核素肾动态显像阳性检出率与彩超检出率对比差异无统计学意义(P>0.05);A组不同程度肾损伤的GFR值呈现为轻度损伤>中度损伤>重度损伤>极重度损伤,组间差异有统计学意义(P<0.05)。结论采用核素肾动态显像可以获得肾积水患者的GFR值,从而评估患者的肾功能损伤程度,不仅操作比较简单,而又具有较好的可重复性,临床可以将其作为此类患者肾功能评估的重要手段。Objective To study the clinical value of nuclide dynamic renal imaging in the evaluation of renal function in patients with hydronephrosis.Methods Selected 96 cases, From January 2020 to January 2021, our hospital patients receiving kidney seeper, the renal biopsy colostomy surgery patients with confirmed the presence of functional, active almighty as A group in 50 cases, renal failure, 46 cases as group B, then using color doppler ultrasonic diagnostic instrument within the group 2 patients had renal blood flow resistance index(RI) were measured, The glomerular filtration rate(GFR) of the patients was measured by dynamic nuclide renal imaging, and the RI and GFR test results of the two groups were compared.Results The renal blood flow resistance index RI in group B was significantly higher than that in group A, P<0.05;The GFR of group B was significantly lower than that of group A(P<0.05). There was no significant difference in the positive rate of dynamic radionuclide renal imaging(P bB0 0.05) between the two groups compared with the color ultrasound. The GFR values of different degrees of kidney injury in group A showed mild injury > moderate injury > severe injury > extremely severe injury, with significant differences between groups(P<0.05).Conclusion Nuclide dynamic renal imaging can be used to obtain the GFR value of patients with hydronephrosis, so as to evaluate the degree of renal function injury in patients. It is not only simple to operate, but also has good reproducibility, and can be used as an important means of renal function assessment in such patients clinically.
