血清脂蛋白-a与同型半胱氨酸和脂蛋白相关磷脂酶A2联合检测在冠心病中的应用

摘　　要：目的分析冠心病患者联合应用血清脂蛋白a[Lp(a)]、同型半胱氨酸(Hcy)联合脂蛋白相关磷脂酶A2(Lp-PLA2)检测的临床效果。方法选取86例濮阳市油田总医院2019年4月—2020年5月收治的冠心病患者为研究组,其中稳定型心绞痛30例,不稳定型心绞痛30例和急性心肌梗死26例,另选取50例同期健康体检者为对照组,比较各组研究对象的Lp(a)、Hcy以及Lp-PLA2水平,并分析Lp(a)、Hcy以及Lp-PLA2联合诊断冠心病的效果。结果研究组患者的Lp(a)、Hcy及Lp-PLA2水平高于对照组,差异有统计学意义(P<0.05)。急性心肌梗死患者的Lp(a)、Hcy及Lp-PLA2水平高于稳定型、不稳定型心绞痛患者,且不稳定型心绞痛患者的Lp(a)、Hcy以及Lp-PLA2水平高于稳定型心绞痛患者,差异有统计学意义(P<0.05)。Lp(a)、Hcy联合Lp-PLA2诊断冠心病的符合率83.09%,敏感度为94.19%,与各指标单独检测结果比较差异有统计学意义(P<0.05)。结论Lp(a)、Hcy与Lp-PLA2联合检测,能够有效提升临床对冠心病的诊断准确率,具有较高推广应用价值。Objective To analyze the clinical effect of serum lipoprotein a[LP(a)],homocysteine(Hcy)combined with lipoprotein associated phospholipase A2(Lp-PLA2)detection in patients with coronary heart disease.Methods Chose 86 cases of our hospital from April 2019 to May 2020 were analyzed for patients with coronary heart disease group,30 cases were divided into stable angina,30 cases of unstable angina and acute myocardial infarction 26 cases,the other selected 50 cases of our hospital health checks for a control during this period,the comparison between groups research object of Lp(a),Hcy and Lp-the level of PLA2,and analyze the Lp,Hcy and Lp(a)-PLA2 in combination with the effect of the diagnosis of coronary heart disease(CHD).Results The levels of Lp(a),Hcy and Lp-PLA2 in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).The levels of Lp(a),Hcy and Lp-PLA2 in patients with acute myocardial infarction were higher than those in patients with stable and unstable angina pectoris,and the levels of Lp(a),Hcy and Lp-PLA2 in patients with unstable angina pectoris were higher than those in patients with stable angina pectoris,the differences were significant(P<0.05).The coincidence rate and sensitivity of Lp-PLA2 combined with Lp(a)and Hcy in the diagnosis of CHD were 83.09%and 94.19%,which were significantly different from the results of each indicator alone(P<0.05).Conclusion The combined detection of Lp(a),Hcy and Lp-PLA2 can effectively improve the diagnostic accuracy of coronary heart disease,and has high popularization and application value.
