摘　　要：目的利用CT冠状动脉成像测量心脏周围脂肪体积,探讨心脏周围脂肪与冠心病的相关性。方法选取2017年1月至2019年12月行CT冠状动脉成像和冠状动脉造影确诊为冠心病的患者152例,将重组图像传至后处理工作站测量心脏周围脂肪体积(包括心周脂肪体积、心外膜脂肪体积、心包膜外脂肪体积),记录冠心病患者斑块性质、钙化积分、血管病变数量、狭窄程度、心功能等级资料。结果心外膜脂肪体积、心周脂肪体积与钙化积分呈正相关(r=0.654,P<0.001;r=0.524,P<0.001);心包膜外脂肪体积与钙化积分没有相关性(r=0.074,P=0.366)。心外膜脂肪体积、心周脂肪体积、心包膜外脂肪体积在不同性质斑块之间差异有统计学意义(F=84.498,P<0.001;F=53.927,P<0.001;F=3.352,P=0.038)。心外膜脂肪体积、心周脂肪体积与病变数量呈正相关(r=0.342,P<0.001,r=0.339,P<0.001);心包膜外脂肪体积与病变数量没有相关性(r=0.109,P=0.183)。心外膜脂肪体积、心周总脂肪体积、心包膜外脂肪体积与血管狭窄程度呈正相关(r=0.770,P<0.001;r=0.687,P<0.001;r=0.229,P=0.005)。心外膜脂肪体积、心周总脂肪体积与心功能等级呈正相关(r=0.406,P<0.001;r=0.377,P<0.001);心包膜外脂肪体积与心功能等级无相关性(r=0.108,P=0.184)。结论心周脂肪体积、心外膜脂肪体积、心包膜外脂肪体积与斑块性质、钙化积分、血管病变数量、狭窄程度、心功能等级之间有相关性,且心外膜脂肪体积相关性较高。Objective To investigate the relationship between Pericardial adipose tissue volume and coronary heart disease by CT coronary angiography.Methods A retrospective study of 152 patients from January 2017 to December 2019 who were identified as coronary heart disease underwent 256-slice spiral CT coronary angiography and coronary angiography.The pericardial fat volume were measured by 3 D postprocessing workstation,and recorded plaque characters,calcification score,lesion count,coronary artery stenosis degree,cardiac function grade.Results There was a positive correlation between Epicardial adipose tissue volume(EATV),Pericardial adipose tissue volume(PATV)and calcification score(r=0.654,P<0.001;r=0.524,P<0.001);There was no correlation between paracardial adipose tissue volume and calcification score(r=0.074,P=0.366).There was a statistically significance in EATV,PATV,and paracardial adipose tissue volume between different plaque characters(F=84.498,P<0.001;F=53.927,P<0.001;F=3.352,P=0.038).There was a positive correlation between EATV,PATV and lesion count(r=0.342,P<0.001;r=0.339,P<0.001);There was no correlation between paracardial adipose tissue volume and lesion count(r=0.109,P=0.183).There was a positive correlation between EATV,PATV,paracardial adipose tissue volume and coronary artery stenosis degree(r=0.770,P<0.001;r=0.687,P<0.001;r=0.229,P=0.005).There was a positive correlation between EATV,PATV and cardiac function grade(r=0.406,P<0.001;r=0.377,P<0.001);There was no correlation between paracardial adipose tissue volume and cardiac function grade(r=0.108,P=0.184).Conclusion EATV,PATV,paracardial adipose tissue volume is correlated with plaque characters,calcification score,lesion count,coronary artery stenosis degree,cardiac function grade.The correlation between EATV is higher than others.
