经导管弹簧圈栓塞以咯血为主要症状的叶内型肺隔离症6例临床分析并文献复习

摘　　要：目的探讨经导管弹簧圈栓塞以咯血为主要症状的叶内型肺隔离症(PS)的临床疗效并总结临床经验。方法回顾性分析2018年3月至2020年1月行导管弹簧圈栓塞的6例叶内型PS患者的临床资料,6例均以咯血为主要症状,伴咳嗽、咳痰4例,胸痛1例。并对国内外相关文献进行综述。结果造影显示6例PS均为单支迷走动脉供血。1例起自腹主动脉供应右下肺基底段,5例起自胸主动脉均供应左下肺基底段。所有患者均应用弹簧圈栓塞迷走动脉主干,术后1~3天咯血症状完全消失。随访3~18个月,无咯血复发及肺梗死、弹簧圈异位栓塞等并发症出现。结论弹簧圈栓塞以咯血为主要症状的叶内型PS疗效确切,只需栓塞PS责任供血动脉主干,无需使用颗粒栓塞剂栓塞末梢血管。Objective To investigate the clinical efficacy of transcatheter coil embolization for intralobar pulmonary sequestration with hemoptysis as the main symptom and summarize the clinical experience.Methods The clinical data of 6 patients with intralobar pulmonary sequestration treated from March 2018 to January 2020 were retrospectively analyzed,and the relevant literatures at home and abroad were reviewed.Results The angiographic results showed six vagal feeding arteries,all of which were supplied by a single vagal artery.One case originated from the abdominal aorta supplying the right lower lung basal segment,and five cases originated from the thoracic aorta supplying the left lower lung basal segment.All patients used coils to embolize the main vagal artery,and the symptoms of hemoptysis disappeared completely 1~3 days after operation.Following up for 3~18 months showed no recurrence of hemoptysis and complications.Conclusion Coil embolization for intralobar pulmonary isolation with hemoptysis as the main symptom has a definite curative effect,with only embolizing the trunk of the abnormal feeding artery and no particle embolic agent for embolizing the peripheral vessels.
