摘 要:目的探讨最优化采集可变翻转角三维快速自旋回波(3D-SPACE)与半傅里叶采集单次激发二维快速自旋回波(2D-HASTE)在磁共振胰胆管成像(MRCP)中的应用对比。方法使用Siemens Aera 1.5 T磁共振扫描仪对81名受检者同时采用3D-SPACE与2D-HASTE序列行MRCP检查。将采集到的图像利用Siemens工作站后处理软件进行重组,由两名高年资医师对成像效果进行评价和评分,评分结果采用非参数检验和χ^(2)检验进行统计学分析。结果在原始图像上所测得的3D-SPACE和2D-HASTE的胆总管信噪比(SNR)差异具有统计学意义(P=0.000),3D-SPACE的胆总管平均SNR(十二指肠上段70.40,十二指肠后段70.80,胰腺段61.30)高于2D-HASTE(十二指肠上段36.60,十二指肠后段36.40,胰腺段33.80)。主胰管最大强度投影(MIP)评分结果差异具有统计学意义(P=0.000),胆总管、左、右肝内胆管评分结果、MRCP质量评分和伪影评分结果均无统计学差异(P>0.05)。结论在1.5 T MRCP中,3D-SPACE序列的成像质量优于2D-HASTE序列,可作为MRCP常规扫描序列之一。Objective To compare the application of 3-dimensional sampling perfection with application optimized contrasts using different flip angle evolutions(3 D-SPACE)sequence and 2-dimensional Half-Fourior Acpuisition Sing-shot Turbo spin Echo(2 D-HASTE)in Magnetic Resonance Cholangiopancreatography(MRCP)examination.Methods 81 patients undergone MRCP scanning using Siemens Aera 1.5 T magnetic resonance imaging(MRI)scanner.Image acquisition was performed on the workstation of Siemens equipped with post-processing software.The images were evaluated and scored by two senior radiologists separately.The scoring results were analyzed statistically with non-parametric analysis and χ^(2) analysis.Results SNRs from original images of 3 D-SPACE and 2 D-HASTE had statistical difference(P=0.000),the mean SNR of common bile duct using 3 D-SPACE(Upper segment of duodenum 70.40,Posterior segment of duodenum 70.8,pancreatic segement 61.30)was higher than the mean SNR using 2 D-HASTE(Upper segment of duodenum 36.60,Posterior segment of duodenum 36.40,pancreatic segement 33.80).The scores of main pancreatic duct had statistical difference(P=0.000),while scores of common bile duct,right and left hepatic ducts,quality of MRCP images and artifact had no statistical difference(P>0.05).Conclusion The quality of images from 3 D-SPACE was better than images from 2 D-HASTE.3 D-SPACE sequence should be carried out in routine MRCP examinations.
