细辛脑注射液、布地奈德联合异丙托溴铵吸入治疗成人哮喘的疗效

摘　　要：目的观察细辛脑注射液、布地奈德联合异丙托溴铵吸入治疗成人哮喘的临床效果。方法选取2018年1月—2020年1月灌南县人民医院呼吸内科成人哮喘患者90例,按照随机数字表法分为对照组和观察组各45例,对照组给予布地奈德联合异丙托溴铵吸入治疗,观察组在对照组基础上给予细辛脑注射液静脉滴注。对比两组的临床疗效、症状体征消失时间、肺功能、SaO_(2)、SF-36评分、气道炎症指标变化。结果在总有效率方面,观察组的95.56%显著高于对照组的80.00%(χ^(2)=5.075,P<0.05);观察组患者的气喘消失时间,肺部湿啰音、哮鸣音消失时间,咳嗽消失时间均短于对照组(P<0.05);观察组患者治疗后较治疗前的FEV1、FVC、PEF升高幅度均高于对照组(P<0.05);观察组患者治疗后较治疗前的RR降低幅度,SaO_(2)、SF-36评分升高幅度均较对照组明显;与对照组相比,观察组患者气道炎症指标S1P、TNF-α显著降低(P<0.05)以上各项指标的比较,差异均具有统计学意义。结论成人哮喘应用细辛脑注射液、布地奈德联合异丙托溴铵吸入治疗的临床效果确切,更利于促进症状的缓解,提高肺功能、生活质量,降低气道炎症,值得临床推广。Objective:To observe the clinical effect of asarinol injectionand budesonide combined with ipratropium bromide inhalation in the treatment of adult asthma.Methods:Ninety adult patients with asthma in the Respiratory Departmentof Guannan County People's Hospital from January 2018 to January 2020 were selected and divided into control group(n=45)and observation group(n=45)according to random number table method.The control group was given budesonide combined with ipratropium bromide inhalation treatment,and the observation group was given asarinol injection intravenous infusion on the basis of the control group.The clinical efficacy,disappearance time of symptoms and signs,lung function,SaO_(2),SF-36 score and airway inflammation indexes were compared between the two groups.Results:In terms of the total effective rate,95.56%in the observation group was significantly higher than 80.00%in the control group(χ^(2)=5.075,P<0.05).The time of asthma disappearance,rales,wheezing and cough disappearance in observation group were significantly shorter than those in control group(P<0.05).After treatment,the increase rates of FEV1,FVC and PEF in observation group were significantly higher than those in control group(P<0.05).After treatment,RR decreased,SaO_(2) and SF-36 scores increased in the observation group compared with those in the control group,with statistical significance(P<0.05).Compared with the control group,airway inflammatory indexes S1P and TNF-αin observation group were significantly decreased(P<0.05).Conclusion:Asarineol injectionand budesonide combined with ipratropium bromide inhalation treatment of adult asthma has definite clinical effect.It is more conducive to promotion of the relief of symptoms,improvement of lung function and quality of lifeand reduce airway inflammation,which is worthy of clinical promotion.
