摘　　要：目的探讨创伤性脓毒症患者IL-1β、IL-6、IL-18、TNF-α因子变化分析。方法选择就诊的120例创伤患者作为本次的研究对象,参加试验的患者入院时间2018年1月至2019年1月,依据患者病症程度分为对照组、创伤组、创伤性脓毒症组,对3组患者血清IL-1β、IL-6、IL-18、TNF-α因子变化趋势、免疫功能状态的细胞因子以及外周血Th17/Treg平衡相关细胞因子含量进行分析。结果发病后创伤性脓毒症组炎性因子水平高于对照组和创伤组(P<0.05);确诊后1 d IL-6和IL-18水平为上升趋势,确诊后3~7 d为下降趋势;确诊后第3天,TNF-α和IL-1β水平为上升趋势,确诊后5~7 d为下降趋势;创伤后发生脓毒症的患者比未发生脓毒症的患者IL-18水平高,且脓毒症的严重程度与循环IL-18水平相关。结论对患者的IL-1β、IL-6、IL-18、TNF-α因子变化进行检测,对脓毒症进行早期合理治疗,提高患者治疗效果。Objective To investigate the changes of expression levels of IL-1β,IL-6,IL-18,TNF-αin patients with traumatic sepsis.Methods A total of 120 patients with trauma who were treated in our hospital from January 2018 and January 2019 were enrolled in the study,who were divided into control group,trauma group and traumatosepsis group according to the degree of disease.The changes of serum levels of IL-1β,IL-6,IL-18,TNF-αfactor,and the cytokines related with immune function and peripheral blood Th17/Treg balance related cytokines were observed and compared among the three groups.Results After the disease onset the levels of inflammatory factors in traumatic sepsis group were significantly higher than those in control group and trauma group.At 1 day after diagnosis,the levels of IL-6 and IL-18 had an increasing trend,and a downward trend from 3 to 7 days.At 3 day after diagnosis,the levels of TNF-αand IL-1βhad an upward trend,and a downward trend from 5d to 7d.The IL-18 levels in patients who occurred pyohemia were significantly higher than those of patients who had no pyohemia,moreover,the severity of pyohemia was related to the circulating IL-18 levels.Conclusion To detect the changes of IL-1β,IL-6,IL-18 and TNF-αfactors and to take reasonable treatment measures can improve the therapeutic effcts on traumatic sepsis.
