摘 要:目的探讨不同部位体成分与空腹血糖、血压和血脂的关系。方法选择306例健康体检者作为研究对象,其中男161例,女145例,年龄21~65岁。采用双能X线骨密度仪(DXA)检测全身脂肪质量、总瘦体重、各部位脂肪质量及占比、骨矿盐含量和骨密度,分别以血糖和血压水平分组,相关分析采用Pearson相关。结果股骨颈骨密度、总腰椎骨密度随着血糖水平升高而降低,且差异有统计学意义(P<0.01)。总瘦体重、下肢脂肪质量及其所占总脂肪质量比例随血糖升高而减少,差异有统计学意义(P<0.01),且与血糖水平呈负相关(P<0.05);躯干脂肪质量及其所占总脂肪质量比例随血糖升高而增加,差异有统计学意义(P<0.01),且与血糖水平呈正相关(P<0.05)。总瘦体重、下肢脂肪质量/总脂肪质量(%)对照组均高于高血压组,差异均有统计学意义(P<0.01),且与血压呈负相关(P<0.05);体脂率、躯干脂肪质量、躯干脂肪质量/总脂肪质量(%)对照组低于高血压组,差异均有统计学意义(P<0.01),且与血压呈正相关(P<0.05)。结论大量的躯干脂肪质量及其比例是心血管代谢性疾病的危险因素,低下肢脂肪质量及其比例亦是。体成分测定或可成为心血管病一级预防的有效手段。Objective To investigate the correlation between body composition and fasting blood glucose,blood pressure,serum lipids in healthy subjects in Qinhuangdao city.Methods A total of 306 healthy subjects with 21~65 years of age,including 161 males and 145 females,were enrolled in the study.The dual-energy X-ray absorptiometry(DXA)was used to detect the total body fat mass(FM),lean mass(LM),and the fat quality and proportion in different positions,bone mineral density(BMD),bone mineral content(BMC).Moreover these patients were divided into three groups according to FBG and two groups according to BP.Results The femoral neck BMD,total lumbar BMD were decreased with the increase of FBG levels(P<0.01),and the LBM,thigh fat mass(TFM)and the proportion of FM were decreased with the increase of FBG levels(P<0.01),which were negatively correlated with FBG(P<0.05).Moreover the trunk fat mass(TrFM)and the percent of FM were increased with the increase of FBG levels(P<0.01),which were positively correlated with FBG(P<0.05).In addition LM,TFM/FM%in control group were significantly higher than in those in hypertension group(P<0.01),which were negatively correlated with BP(P<0.05).Furthermore FM%,TrFM,TrFM/FM%in control group were significantly lower than those in hypertension group(P<0.01),which were positively correlated with BP(P<0.05).Conclusion A lower amount of trunk fat mass and its proportion are risk factors of cadiovascular metabolic diseases,so are the lower limb fat and its proportion,and the detection of body composition may become an effective method for the primary prevention of cardiovascular diseases.
