新型冠状病毒肺炎重大疫情下神经科感染防控应急管理实践

摘　　要：目的探讨新型冠状病毒感染肺炎(简称“新冠肺炎”)期间神经内科病区的感染防控应急管理实践策略。方法根据新冠肺炎的流行病学特点,及国家近期印发的关于该类疾病的相关防控方案、指引和感染防控的相关法律、法规,结合医院感控防控管理原则及神经内科工作特点,及时制定和更新专科诊疗流程、护理流程、病房环境物品管理、患者及陪人管理、医务人员培训及应急人力管理等措施,加强医生护士操作防控培训及疑似新冠患者的防控管理,进行优先的管理策略。结果结合新冠肺炎疾病特点修改医疗流程26项,其中护理流程3项,神经专科诊疗及救治流程3项。组织线上培训医务人员达98人次,线上考试全员以100分通过,知识掌握率100%。83例神经内科患者得到及时有效救治,特别是23例急性卒中患者通过急诊绿色通道得到有效救治,其中16例急性卒中患者通过静脉溶栓恢复正常肌力,5例急诊取栓和2例颅内支架置入术抢救了患者生命,改善患者的生活质量,提高了卒中绿色通道患者的有效救治率,同时保障了医护人员的身体健康。结论在医院防治新冠肺炎的特殊时期,及时收治神经内科脑血管疾病患者,特别是急性卒中患者绿色通道的有效开通,提高了卒中患者的生存质量,降低致残率,同时保护了医务工作者的身体健康,确保新冠肺炎疫情下卒中患者的有效救治。Objective To explore the practical strategies of emergency management for infection prevention and control in the department of neurology during the period of new pneumonia.Methods According to will be coronavirus pneumonia(hereinafter referred to as“COVID-19”)the epidemiological characteristics of,and the recently issued by the related about the disease prevention and control of the plan,guide and infection prevention and control of the relevant laws and regulations,and combining with the management of prevention and control of hospital infection control principles and neurologist working characteristics,and timely to develop and update the diagnosis and treatment process,the nursing process,ward environment item management,patients and with people management,staff training and emergency manpower management,measures such as strengthening the prevention and control of doctors and nurses operating training and the prevention and control of patients with suspected new champions league management,priority of management strategy.Results According to the characteristics of COVID-19 disease,26 items of medical procedures were modified,including 3 items of nursing procedures and 3 items of diagnosis,treatment and treatment procedures in neurosurgery.I organized online training for medical personnel up to 98 times,and all of them passed the online examination with 100 points,with a knowledge mastery rate of 100%.83 neurology patients get timely and effective treatment,especially in 23 cases of acute stroke patients with emergency green channel to get effective treatment,including 16 cases of acute stroke patients by intravenous thrombolysis restored to normal muscle,5 cases of emergency should save the patients life and 2 cases of intracranial carotid stenting,improve the quality of life of patients,improve the green channel effective cure rate of patients with stroke.At the same time to protect the health of medical staff.Conclusion The prevention and control of hospital COVID-19 special period,timely treated neurol
