摘 要:目的对广州市黄埔地区妇女感染人乳头瘤病毒基因亚型的分布情况的统计,为临床医生做进一步的诊治提供一定的参考依据。方法收集2020年6月—2021年3月广州开发区医院西院区门诊、住院和体检一共6181例女性人乳头瘤病毒基因分型标本,采用PCR+膜杂交法进行21种基因分型检测。分析不同年龄段的妇女感染人乳头瘤病毒的阳性情况以及不同亚型的感染情况,并分析高危型HPV和非高危型HPV基因亚型在不同年龄段的感染分布情况。结果①在6181名妇女中,平均年龄约为(39.64±8.34)岁,其中检测结果阳性有618名,阳性率约10.0%,阳性人群平均年龄约为(39.20±9.61)岁;②阳性案例中按年龄分组情况:≤20岁(n=7,1.13%);21~30岁(n=110,17.8%);31~40岁(n=238,38.51%);41~50岁(n=185,29.94%);51~60岁(n=67,10.84%);>60岁(n=11,1.78%);③感染率最高的前三个亚型分别为高危型52(n=151,24.43%)、高危型58(n=89,14.40%)、其他型53(n=68,11.0%);④其中单一感染(n=505,81.7%),双重感染(n=97,15.7%),三重及以上多重感染(n=16,2.6%)。结论广州市黄埔地区检测人群中HPV感染以高危型52亚型为主。30~50岁作为中青年的这个年龄段占一半以上的比例,而HPV高危型持续感染作为导致宫颈癌的重要原因之一。了解该地区HPV的基因分型的情况及年龄的分布的情况,可为临床指导用药及疫苗选择提供一定的参考作用。Objective To investigate the distribution of Human papillomavirus genotypes among women in Huangpu District of Guangzhou.To provide certain reference basis for clinicians to make further diagnosis and treatment.Methods A total of 6181 female human papillomavirus genotyping specimens were collected from June 2020 to March 2021 in the outpatient,inpatient and physical examination of the West District of Guangzhou Development Zone Hospital,and 21 types of genotyping were detected by PCR+membrane hybridization.Analyze the positive status of human papillomavirus infection in women of different age groups and the infection status of different subtypes,and analyze the infection distribution of high-risk HPV and non-high-risk HPV gene subtypes in different age groups.Results①Among the 6181 women,the average age was about(39.64±8.34)years old,of which 618 had a positive test result,the positive rate was about 10.0%,and the average age of the positive population was about(39.20±9.61)years old;②According to the positive cases Age group:≤20 years old(n=7,1.13%);21-30 years old(n=110,17.8%);31-40 years old(n=238,38.51%);41-50 years old(n=185,29.94%);51-60 years old(n=67,10.84%);>60 years old(n=11,1.78%);③The first three subtypes with the highest infection rate are high-risk 52(n=151,24.43%),high-risk 58(n=89,14.40%),other 53(n=68,11.0%);④single infection(n=505,81.7%),double infection(n=97,15.7%),triple and above multiple infections(n=16,2.6%).Conclusion HPV infections in the tested population in Huangpu area of Guangzhou are mainly high-risk 52 subtypes.The age group of 30-50 years old as young and middle-aged accounts for more than half of the proportion,and high-risk HPV persistent infection is responsible for as one of the important causes of cervical cancer.To understand the genotyping and age distribution of HPV in this area can provide reference for clinical medication and vaccine selection.
