瑞芬太尼联合右美托咪定在蛛网膜下腔出血患者术后镇痛镇静中的应用

摘　　要：目的探讨瑞芬太尼联合右美托咪定在蛛网膜下腔出血患者术后镇痛镇静中的应用效果。方法根据术后镇痛镇静方案不同将我院收治的96例蛛网膜下腔出血患者分为两组。对照组(38例)术后采用瑞芬太尼进行镇痛镇静,观察组(58例)术后采用瑞芬太尼联合右美托咪定进行镇痛镇静。比较两组患者的镇静、镇痛效果。结果给药6 h、12 h、24 h后,两组患者的Ramsay评分均高于给药前,CPOT评分均低于给药前,且观察组的Ramsay评分高于对照组,CPOT评分低于对照组(P<0.05)。结论蛛网膜下腔出血患者术后采用瑞芬太尼联合右美托咪定进行镇痛镇静,可提高其镇静效果,缓解患者的疼痛。Objective To explore the application effect of remifentanil combined with dexmedetomidine in postoperative analgesia and sedation of patients with subarachnoid hemorrhage.Methods 96 cases of patients with subarachnoid hemorrhage admitted to our hospital were divided into two groups according to different postoperative analgesia and sedation programs.The control group(38 cases)was given remifentanil for postoperative analgesia and sedation,and the observation group(58 cases)was given remifentanil combined with dexmedetomidine for postoperative analgesia and sedation.The sedative and analgesic effects of the two groups were compared.Results 6 h,12 h and 24 h after administration,the Ramsay scores of the two groups were higher than those before administration,and the CPOT scores were lower than those before administration(P<0.05);The Ramsay scores of the observation group were higher than those of the control group,and the CPOT scores were lower than those of the control group(P<0.05).Conclusions Remifentanil combined with dexmedetomidine for postoperative analgesia and sedation of patients with subarachnoid hemorrhage can improve the sedative effect,and relieve the pain.
