不同剂量左旋甲状腺素钠片治疗妊娠合并甲减的临床效果分析

摘　　要：目的分析不同剂量左旋甲状腺素钠片治疗妊娠合并甲减的临床效果。方法选取2018年5月至2020年4月我院收治的妊娠合并甲减患者86例,随机分为对照组和观察组各43例。对照组采用低剂量左旋甲状腺素钠片治疗,观察组采用高剂量左旋甲状腺素钠片治疗。比较两组的临床疗效、治疗前后甲状腺功能及不良妊娠结局发生情况。结果观察组治疗总有效率为95.35%,明显高于对照组的76.74%(P<0.05)。治疗后,两组的TSH水平低于治疗前,FT3、FT4水平高于治疗前,且观察组的TSH水平低于对照组,FT3、FT4水平高于对照组(P<0.05)。观察组的不良妊娠结局发生率为4.65%,明显低于对照组的18.60%(P<0.05)。结论与低剂量左旋甲状腺素钠片治疗相比,高剂量左旋甲状腺素钠片治疗妊娠合并甲减临床效果更好,可有效改善患者的甲状腺功能,降低不良妊娠结局发生率。Objective To analyze the clinical effect of different doses of levothyroxine sodium tablets in the treatment of pregnancy complicated with hypothyroidism.Methods 86 pregnancy patients with hypothyroidism admitted to our hospital from May 2018 to April2020 were selected and randomly divided into control group and observation group,with 43 cases in each group.The control group was treated with low-dose levothyroxine sodium tablets,and the observation group was treated with high-dose levothyroxine sodium tablets.The clinical efficacy,thyroid function before and after treatment and the adverse pregnancy outcomes were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.35%,significantly higher than 76.74%in the control group(P<0.05).After treatment,the TSH levels of the two groups were lower than those before treatment,and the FT3 and FT4 levels were higher than those before treatment(P<0.05);The TSH level of the observation group was higher than that of the control group,and the FT3 and FT4 levels were higher than those of the control group(P<0.05).The incidence of adverse pregnancy outcomes in the observation group was4.65%,significantly lower than 18.60%in the control group(P<0.05).Conclusions Compared with low-dose levothyroxine sodium tablets,high-dose levothyroxine sodium tablets in the treatment of pregnancy complicated with hypothyroidism has better clinical effect,which can effectively improve the thyroid function of patients,and reduce the incidence of adverse pregnancy outcomes.
