血浆置换联合胆红素吸附治疗乙型肝炎肝衰竭的临床效果探讨

摘　　要：目的探讨血浆置换(PE)联合胆红素吸附(BA)治疗乙型肝炎肝衰竭的临床效果。方法将45例乙型肝炎肝衰竭按治疗方法不同分为对照组(n=22)和观察组(n=23)。两组均采用常规治疗。对照组采用PE治疗,观察组采用PE联合BA治疗。比较两组患者治疗前及治疗4周后的肝功能指标[谷丙转氨酶(ALT)、谷氨酰转肽酶(GGT)、总胆红素(TBIL)]以及并发症发生情况。结果治疗4周后,两组的血清ALT、GGT、TBIL水平低于治疗前,且观察组的血清ALT、GGT、TBIL水平低于对照组(P<0.05)。两组患者治疗期间的并发症发生率(8.70%vs.4.55%)比较差异无统计学意义(P>0.05)。结论PE联合BA治疗乙型肝炎肝衰竭效果显著,可明显改善患者的肝功能,安全性较高,值得临床推广应用。Objective To explore the clinical effect of plasma exchange(PE)combined with bilirubin adsorption(BA)in the treatment of hepatitis B liver failure.Methods 45 patients with hepatitis B liver failure were divided into control group(n=22)and observation group(n=23)according to different treatment methods.Both groups received conventional treatment.The control group was treated with PE,and the observation group was treated with PE combined with BA.The liver function indicators[alanine aminotransferase(ALT),glutamyl transpeptidase(GGT),total bilirubin(TBIL)]before treatment and after 4 weeks of treatment,and the complications were compared between the two groups.Results After 4 weeks of treatment,the ALT,GGT and TBIL levels of both groups were lower than those before treatment,and the ALT,GGT and TBIL levels of the observation group were lower than those of the control group(P<0.05).During treatment,no statistical difference was found in the incidence of complications(8.70%vs.4.55%)between the two groups(P>0.05).Conclusions PE combined with BA in the treatment of hepatitis B liver failure has significant effect,and can significantly improve the liver function of patients,with higher safety,which is worthy of clinical promotion and application.
