无临床心脏受累结节病患者左心室整体纵向应变的变化

摘　　要：目的:探讨在无临床心脏受累的结节病患者中通过二维超声心动图斑点追踪测定的左心室整体纵向应变(GLS)的变化。方法:回顾性连续入选2000-01—2020-09于北京大学第一医院住院并行超声心动图的结节病患者,排除已诊断心脏结节病或存在其他心脏病、心脏症状/体征的患者,共入选48例。按1∶1选取无心脏病的对照者,匹配性别、年龄及高血压与糖尿病史。通过二维斑点追踪测定上述患者和对照者的左心室GLS。结果:结节病患者的年龄为(55±10)岁,男性15例(31.3%)。结节病患者和对照者的左房、左室大小、左心室射血分数及左心室舒张功能指标之间均差异无统计学意义。结节病患者和对照者的左心室GLS分别为(-17.9±3.4)%和(-23.4±3.7)%,差异有统计学意义(P<0.05)。结论:无临床心脏受累的结节病患者左心室GLS明显受损。左心室GLS可能是结节病患者亚临床心脏受累的标志之一。Objective:To investigate the changes of left ventricular global longitudinal strain(GLS)measured by two-dimensional echocardiographic speckle tracking in sarcoidosis patients without apparent cardiac involvement.Methods:Patients who were admitted to Peking University First Hospital and diagnosed with sarcoidosis from January 2000 to September 2020 were enrolled retrospectively and consecutively.Those who were diagnosed with cardiac sarcoidosis,had other heart diseases or cardiac symptoms/signs were excluded.A total of 48 patients were finally included.The controls without heart diseases were matched by 1∶1 according to sex,age,and hypertension or diabetes mellitus history.The left ventricular GLS of the included patients and the controls were measured by two-dimensional echocardiographic speckle tracking.Results:The age was 55±10 years old and 15(31.3%)were male in patients with sarcoidosis.There was no statistically significant difference in left atrial size,left ventricular size,left ventricular ejection fraction,and left ventricular diastolic function indicators between patients with sarcoidosis and the controls.The left ventricle GLS of sarcoidosis patients and the controls was(-17.9±3.4)%and(-23.4±3.7)%,respectively,and the difference was statistically significant(P<0.05).Conclusion:Left ventricular GLS is significantly impaired in sarcoidosis patients without apparent cardiac involvement.Left ventricular GLS may be one of the markers of subclinical cardiac involvement in patients with sarcoidosis.
