摘　　要：目的探讨一期后路病灶清除植骨内固定术对脊柱结核患者腰骶角、椎间隙高度及炎性因子水平的影响。方法选取2016年7月至2020年7月收治的脊柱结核患者80例,采用抽签法以单双号分组形式将其分为对照组和观察组,各40例。对照组采用前后联合入路病灶清除植骨内固定术,观察组采用一期后路病灶清除植骨内固定术。比较两组的手术效果。结果观察组的手术时间、住院时间短于对照组,术中出血量少于对照组,术后VAS评分低于对照组(P<0.05)。术后,两组的腰骶角、椎间隙高度均较术前增大(P<0.05)。术后,观察组的CRP水平、ESR低于对照组(P<0.05)。结论一期后路病灶清除植骨内固定术治疗脊柱结核有助于缩短手术时间,减少术中出血量,减轻术后疼痛,加快恢复效率,值得推广。Objective To explore the effects of one-stage posterior debridement and bone grafting and internal fixation on the lumbosacral angle,intervertebral space height and the levels of inflammatory factors in patients with spinal tuberculosis.Methods A total of 80 patients with spinal tuberculosis admitted from July 2016 to July 2020 were selected and divided into control group and observation group by drawing lots in the form of single and double numbers,with 40 cases in each group.The control group was treated with combined anterior and posterior approach for debridement and bone grafting and internal fixation,and the observation group was treated with one-stage posterior debridement and bone grafting and internal fixation.The operation effects of the two groups were compared.Results The operation time and hospital stay in the observation group were shorter than those in the control group,the amount of intraoperative bleeding was less than that in the control group,and the postoperative VAS score was lower than that in the control group(P<0.05).After operation,the lumbosacral angle and intervertebral space height of the two groups increased compared with those before operation(P<0.05).After operation,CRP level and ESR in the observation group were lower than those in the control group(P<0.05).Conclusion One-stage posterior debridement and bone grafting and internal fixation in the treatment of spinal tuberculosis is helpful to shorten the operation time,reduce the amount of intraoperative bleeding and postoperative pain,and accelerate the recovery efficiency,which is worthy of popularization.
