摘　　要：目的探讨右美托咪定超前镇痛对腹腔镜下子宫肌瘤剔除术患者应激反应及血流动力学指标的影响。方法选取2019年7月至2020年10月收治的70例腹腔镜下子宫肌瘤剔除术患者,以抽签法随机将其分为对照组和观察组,各35例。对照组采用常规麻醉,观察组在常规麻醉基础上予以右美托咪定超前镇痛,术后两组均实施自控镇痛。比较两组的镇痛效果。结果术后4、12、24及48 h,观察组的VAS评分均低于对照组(P<0.05)。术中、术后24 h,观察组的Cor、NE水平均低于对照组(P<0.05)。术中、术后24 h,观察组的收缩压、舒张压、心率均低于对照组(P<0.05)。结论右美托咪定超前镇痛用于腹腔镜下子宫肌瘤剔除术中能够有效减轻患者术后的疼痛程度及应激反应,稳定血流动力学指标,值得推广。Objective To explore the effects of dexmedetomidine preemptive analgesia on stress response and hemodynamics indexes in patients undergoing laparoscopic myomectomy.Methods A total of 70 patients with laparoscopic myomectomy admitted from July 2019 to October 2020 were selected and randomly divided into control group and observation group by drawing lots,with 35 cases in each group.The control group was given routine anesthesia,and the observation group was given dexmedetomidine preemptive analgesia on the basis of routine anesthesia.Both groups were given patient-controlled analgesia after operation.The analgesic effects of the two groups were compared.Results At 4,12,24 and 48 h after operation,the VAS score of the observation group were lower than those of the control group(P<0.05).During operation and at 24 h after operation,the levels of Cor and NE in the observation group were lower than those in the control group(P<0.05).During operation and at 24 h after operation,the systolic pressure,diastolic pressure and heart rate in the observation group were lower than those in the control group(P<0.05).Conclusion Dexmedetomidine preemptive analgesia applied in laparoscopic myomectomy can effectively alleviate the degree of postoperative pain and stress response,and stabilize hemodynamic indexes,which is worthy of promotion.
