摘 要:目的探究母乳喂养对出院后早产儿肺功能的影响。方法选取厦门大学附属第一医院儿科新生儿病区2018年7月至2019年1月收治的早产儿60例,根据出院后喂养方式不同分为母乳喂养组(n=36)和配方奶喂养组(n=24),采用MasterScreen肺功能仪测定两组纠正月龄1~3月时的肺功能,比较两组潮气量、吸气时间、呼气时间、吸呼比、呼吸频率、达峰时间比、达峰容积比,并分析呼气时间的影响因素。结果两组潮气量、吸气时间、呼吸频率、达峰时间比和达峰容积比比较差异无统计学意义。母乳喂养组呼气时间低于配方奶喂养组,差异有统计学意义(P<0.05)。呼气时间与纠正月龄、支气管肺发育不良显著相关(P<0.05),与胎龄、出生体质量、喂养方式、吸氧时间、无创通气时间、住院时间等无明显相关性。结论母乳喂养早产儿潮气肺功能呼气时间显著短于配方奶喂养,可能与母乳喂养可减轻小气道炎症,改善小气道功能相关。Objective To investigate the effects of breastfeeding on lung function of preterm infants after discharge.Methods 60 premature infants were selected from Pediatric Neonatal Ward of the First Affiliated Hospital of Xiamen University from July 2018 to January 2019.According to the feeding patterns after discharge,they were divided into breast-feeding group(n=36)and formula feeding group(n=24).The MasterScreen pulmonary function meter was used to measure the pulmonary function between the two groups at 1 to 3 months of correction age.The tidal volume,inspiratory time,expiratory time,inhalatory ratio,respiratory rate,peaking time ratio and peaking volume ratio of the two groups were compared,and the influencing factors of expiratory time were analyzed.Results There was no significant difference in tidal volume,inspiratory time,respiratory rate,peak time ratio and peak volume ratio between the two groups.The expiratory time of breast-feeding group was lower than that of formula feeding group,and the difference was statistically significant(P<0.05).Expiratory time was significantly correlated with correction months and bronchopulmonary dysplasia(P<0.05),but there was no significant correlation with gestational age,birth weight,feeding mode,oxygen inhalation time,non-invasive ventilation time,hospital stay,etc.Conclusion The expiratory time of breast feeding premature infants is significantly shorter than that of formula feeding,which may be related to the reduction of small airway inflammation and improvement of small airway function.
