摘　　要：目的观察寻常型银屑病患者外周血单个核细胞端粒酶活性变化,探讨其与寻常型银屑病患者焦虑、抑郁的关系。方法100例寻常型银屑病患者为银屑病组,同期100例体检健康者为对照组。银屑病组于就诊时、对照组于体检日采集外周静脉血,采用实时荧光定量PCR法检测外周血单个核细胞端粒酶活性;2组行医院焦虑抑郁量表(Hospital Anxiety and Depression Scale,HADS)评分,比较HADS-A及HADS-D评分;Pearson相关分析寻常型银屑病患者外周血单个核细胞端粒酶活性与HADS-A、HADS-D评分的相关性;以HADS-A评分≥11分为存在焦虑,HADS-D评分≥11分为存在抑郁,记录银屑病组焦虑、抑郁患者例数,比较银屑病组抑郁与未抑郁、焦虑与未焦虑患者外周血单个核细胞端粒酶活性;绘制ROC曲线,评估外周血单个核细胞端粒酶活性预测寻常型银屑病患者焦虑、抑郁的价值。结果银屑病组外周血单个核细胞端粒酶活性(2.01±0.54)、HADS-A评分[(17.05±3.25)分]、HADS-D评分[(16.97±4.21)分]均高于对照组[0.91±0.12、(6.32±1.14)分、(6.20±1.67)分](P<0.05)。Pearson相关分析结果显示,寻常型银屑病患者HADS-A评分(r=0.745,P=0.003)、HADS-D评分(r=0.643,P=0.004)与外周血单个核细胞端粒酶活性均呈正相关。银屑病组35例存在焦虑,65例存在抑郁,焦虑者外周血单个核细胞端粒酶活性(2.78±0.42)高于未焦虑者(1.73±0.51)(P<0.05),抑郁者外周血单个核细胞端粒酶活性(2.84±0.53)高于未抑郁者(1.73±0.46)(P<0.05)。ROC曲线分析结果显示,外周血单个核细胞端粒酶活性分别以2.42、2.42为最佳截断值,预测寻常型银屑病患者焦虑、抑郁的AUC分别为0.796(95%CI:0.649~0.829,P<0.001)、0.902(95%CI:0.835~0.942,P<0.001),灵敏度分别为84.15%、93.24%,特异度分别为79.28%、95.25%。结论寻常型银屑病患者外周血单个核细胞端粒酶活性增强,与其发生焦虑、抑郁有关,对预测寻常型银屑病患者焦虑�Objective To observe the change of telomerase activity in peripheral blood mononuclear cells in patients with psoriasis vulgaris and to investigate its association with anxiety and depression.Methods The peripheral venous blood was collected at admission in 100 patients with psoriasis vulgaris(psoriasis group)and on the physical examination day in 100 healthy volunteers(control group).The telomerase activity in peripheral blood mononuclear cells was detected by real-time fluorescence quantitative PCR.Hospital Anxiety and Depression Scale(HADS)was used to measure the HADS-anxiety(HADS-A)and HADS-depression(HADS-D)scores.Pearson linear correlation method was used to analyze the correlations of telomerase activity in peripheral blood mononuclear cells with HADS-A and HADS-D scores.When the HADS-A and HADS-D scores were more than 11,the petients were diagnosed with anxiety and depression.The numbers of patients with anxiety and depression in psoriasis group were recorded.The telomerase activity was compared between patients with and without anxiety and depression in psoriasis group.ROC curve was drawn to assess the value of the telomerase activity of peripheral blood mononuclear cells to the prediction of anxiety and depression in psoriasis vulgaris patients.Results The telomerase activity,HADS-A score and HADS-D score were higher in psoriasis group(2.01±0.54,17.05±3.25,16.97±4.21)than those in control group(0.91±0.12,6.32±1.14,6.20±1.67)(P<0.05).Pearson linear correlation analysis showed that HADS-A score(r=0.745,P=0.003)and HADS-D score(r=0.643,P=0.004)were positively correlated with telomerase activity in psoriasis vulgaris patients.In psoriasis group,65 patients developed depression,and 35 patients developed anxiety.The telomerase activity was higher in patients with anxiety(2.78±0.42)than that in patients without anxiety(1.73±0.51)(P<0.05),and higher in patients with depression(2.84±0.53)than that in patients without depression(1.73±0.46)(P<0.05).ROC analysis showed when the optimal cut-offvalue of tel
