T2DM下肢血管病变患者应用Buerger运动结合步行训练的价值

摘　　要：目的:探讨2型糖尿病(T2DM)下肢血管病变患者进行Buerger运动和步行训练的效果。方法:选择2018年3月-2019年10月本院内分泌科收治的68例T2DM伴下肢血管病变的患者,按奇偶数分组法分为步行组(奇数)34例、结合组(偶数)34例。步行组接受单纯步行训练,结合组接受Buerger运动和步行训练。观察两组症状表现、下肢血流情况、周围神经病变。结果:结合组干预后的下肢凉感、静息痛、间歇性跛行症状发生率均低于步行组(P<0.05),踝肱指数(ABI)、足背动脉血流速度(DAMV)高于步行组(P<0.05),多伦多神经病变评分系统量表评分(TCSS)评分低于步行组(P<0.05)。结论:Buerger运动结合步行训练可通过改善T2DM下肢血管病变患者的下肢血液循环,改善周围神经病变,进而缓解下肢凉感等症状表现。Objective:To investigate the effect of Buerger exercise and walking training in patients with lower limb vasculopathy of type 2 diabetes mellitus (T2DM).Method:From March 2018 to October 2019,68 T2DM patients with lower limb vasculopathy were enrolled in the Endocrinology Department of our hospital.According to the odd and even grouping method,34 patients were walking group (odd),34 patients were combined group (even).The walking group received simple walking training,and the combined group received Buerger exercise and walking training.The symptom,lower limb blood flow and peripheral neuropathy of the two groups were observed.Result:After intervention,the incidence of cold sensation of lower extremities,rest pain,intermittent claudication symptoms was lower than that of walking group (P<0.05),the ankle-brachial index (ABI),dorsalis pedis artery blood flow velocity (DAMV) were higher than those of the walking group (P<0.05),the Toronto clinical scoring system (TCSS) was lower than that of the walking group (P<0.05).Conclusion:Buerger exercise combined with walking training can improve lower extremity blood circulation and peripheral neuropathy in patients with T2DM lower extremity vascular disease,thus relieving symptoms such as lower extremity cold sensation.
