摘 要:目的:探究慢性阻塞性肺疾病急性加重期(AECOPD)患者无创通气治疗中呼气末二氧化碳分压(PetCO_(2))与动脉血二氧化碳分压(PaCO_(2))之间的相关性及其临床意义。方法:选取2017年1月至2018年12月在中山市人民医院急诊监护室应用无创通气治疗的45例AECOPD患者为研究对象,测定不同通气时间(5 min,30 min,60 min)时的PetCO_(2)与PaCO_(2);分析不同通气时间PetCO_(2)与PaCO_(2)之间的相关性。结果:患者不同通气时间(5 min、30 min、60 min)PaCO_(2)、PetCO_(2)、PaCO_(2)与PetCO_(2)的差值(ADCO_(2))、潮气量比较,差异均无统计学意义(P>0.05)。AECOPD患者在无创通气30 min和60 min时PetCO_(2)与PaCO_(2)有较好的相关性(r=0.86和r=0.89,P=0.003和P=0.005)。结论:AECOPD患者无创通气中PetCO_(2)与PaCO_(2)有良好的相关性,可应用PetCO_(2)来监测无创正压通气治疗效果。Objective To explore the correlation between the end-of-exhalation carbon dioxide pressure(PetCO_(2))and arterial blood co2 pressure(PaCO_(2))in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and it’s clinical significance.Methods 45 patients with AECOPD who applied noninvasive ventilation therapy in the emergency ward of Zhongshan People’s Hospital from January 2017 to December 2018 were selected as study subjects to determine PetCO_(2) and PaCO_(2) at different ventilation times(5 min,30 min,60 min);The correlation between PetCO_(2) and PaCO_(2) in different ventilation periods was analyzed.Results The differences were not statistically significant(P>0.05)compared with the differences(ADCO_(2))and moisture volume of patients at different ventilation times(ventilation 5 min,ventilation 30 min,ventilation 60 min)PaCO_(2),PetCO_(2),PaCO_(2) and PetCO_(2).Patients with acute exacerbation of chronic obstructive pulmonary disease had a good correlation with PaCO_(2) in noninvasive ventilation of 30 min and 60 min PetCO_(2)(r=0.86,r=0.89,P=0.003,P=0.005).Conclusion PetCO_(2) and PaCO_(2) have good correlation in noninvasive ventilation in patients with AECOPD,and PetCO_(2) can be used to monitor the effect of noninvasive positive pressure ventilation therapy.
