摘　　要：结合腹型过敏性紫癜的临床症状及消化内镜下表现,闫慧敏教授认为其与中医内痈"红、肿、热、痛"的特点高度相似,其气血壅滞、血败肉腐的病机特点也与内痈相符,故可归入"内痈"范畴论治。急性期清热祛湿理气、凉血祛瘀通络,恢复期养血活络、健脾祛湿,并结合其胃镜下表现进行局部微观辨证,内镜下见胃肠黏膜充血肿胀明显者应注意清热消痈,黏膜见多发糜烂、溃疡者,需祛瘀通络、敛疮生肌,镜下见胆汁反流或黏液池浑浊,则需增强清利湿热之功。Considering the clinical symptoms and the endoscopic manifestations of the abdominal-type Henoch-Schonlein purpura(HSP),professor YAN Huimin believes that this disease can be classified into"internal abscesses"as it is highly similar to the"redness,swelling,heat and pain"characteristics of internal abscesses in traditional Chinese medicine,and has similar disease mechanism which were qi and blood congestion and constraint,and blood vanquished and flesh putridity.During acute stage,clearing heat,dispelling damp and regulating qi method is recommended,as well as cooling blood,dispelling stasis and unblocking collaterals;in the convalescence period,nourishing blood,quickening the collaterals,fortifying spleen and dispelling dampness should be performed.The gastroscopy results could be combined for local microscopic syndrome differentiation;if there were obvious gastrointestinal mucosal congestion and swelling,heat-clearing and abscess-resolving method should be applied;for patients with multiple erosions and ulcers in the mucous membrane,it is advised to dispel stasis and unblock collaterals,close sore and engender flesh;regarding bile reflux or turbid mucus pool seen under the microscope,the function to clear damp-heat should be strengthened.
