摘　　要：目的探讨部分脾动脉栓塞术(PSE)对肝炎肝硬化脾功能亢进(脾亢)患者免疫功能的影响。方法收集2016年7月至2019年5月昆明医科大学第一附属医院收治70例肝炎肝硬化并脾亢行PSE术患者,采集外周血分析PSE术后机体细胞免疫及体液免疫的变化。结果(1)脾亢后机体CD3+、CD4+、CD8+、NK及B细胞均低于正常值下限,但CD4+/CD8+值位于正常值范围内。(2)PSE术后1、3、6和12个月CD3+、CD4+、CD8+、B及NK淋巴细胞较术前明显增高(P<0.05),但CD4+/CD8+值仍位于正常值范围内。(3)PSE术后1、3、6和12个月IgA、IgM、IgG较术前无明显变化(P>0.05)。结论脾亢后机体免疫功能是处于平衡状态;PSE术对机体免疫功能(细胞免疫及体液免疫)无明显影响。Objective To investigate the effect of partial splenic embolization(PSE)on the immune function in cirrhosis patients with hypersplenism.Methods A total of 70 patients with hepatitis-related cirrhosis complicated by hypersplenism,who were admitted to the First Affiliated Hospital of Kunming Medical University of China between July 2016 and May 2019 to receive PSE treatment,were collected.Peripheral blood sampling was conducted to analyze the changes in cellular immunity and humoral immunity after PSE.Results(1)After having hypersplenism,the CD3+,CD4+,CD8+,NK and B cell values were below the low limit of normal range,but CD4+/CD8+ value was within the normal range.(2)In one month,3 months,6 months and 12 months after PSE,CD3+,CD4+,CD8+,NK and B cell values were significantly increased when compared with the preoperative data(P<0.05),but CD4+/CD8+ value remained within the normal range.(3)In one month,3 months,6 months and 12 months after PSE,the Ig-A,Ig-M and Ig-G levels showed no obvious changes when compared with the preoperative ones(P>0.05).Conclusion In patients with hypersplenism the body’s immune function is in a balanced condition.PSE has no obvious effect on either cellular immunity or humoral immunity.
