摘  要:股骨髋臼撞击综合征(femoroacetabular impingement syndrome,FAI)合并股骨头颈交界区骨囊肿并不少见,本文通过一例关节镜下治疗股骨髋臼撞击综合征合并股骨头颈交界区骨囊肿的临床病例,综述相关文献并探讨总结该类病例的手术治疗原则。结果显示关节镜下治疗FAI合并股骨头颈交界区骨囊肿能取得满意的短期临床效果。作者认为:对于此类病例,较小的囊肿可直接磨除,并可作为凸轮(Cam)畸形成型的标准。而较大的囊肿,应以清理囊腔为主,但不宜过多磨削以免影响股骨颈的力学情况。另外,建议先进行Cam成型再处理囊肿,避免囊肿影响对周围凸轮畸形的判断,从而导致Cam成型不足。Femoroacetabular impingement syndrome(FAI)combined with bone cysts at the femoral head-neck junction is not uncommon.Here we presented the arthroscopic treatment of a such patient and discussed the principle of surgical treatment for this condition based on literature review.The results showed that arthroscopic treatment of FAI combined with bone cysts at the femoral head-neck junction could achieve satisfactory short-term clinical results.The author believes that for such cases,smaller cysts can be directly removed,and used as a standard as femoral osteoplasty for the Cam deformity.For larger cysts,the cyst cavity should be cleaned without too much grinding to avoid affecting the mechanics of the femoral neck.Moreover,it is suggested that femoral osteoplasty should be performed before cyst debridement,so as to avoid insufficient resection due to the wrong evaluation of the Cam deformity.
