摘 要:目的观察Narcotrend脑电/意识麻醉深度监测系统在体外循环术后镇静中的应用效果。方法抽取南昌市某综合性三级甲等医院胸外ICU 2018年4月—2020年4月体外循环下行心脏直视手术患者200例,随机分为试验组及对照组各100例。2组患者术后都携带气管导管回ICU与多功能呼吸机连接,按医嘱采用丙泊酚等持续镇静,在使用镇静剂期间,每隔30 min记录血压、心率、SpO_(2)、呼吸频率结果。试验组采取Narcotrend脑电/意识麻醉深度监测系统对麻醉深度进行监测,对照组采用Ramsay镇静深度评分系统,比较2组患者停药后唤醒时间及拔管时间,ICU停留时间,患者血流动力学指标(收缩压、舒张压、心率、呼吸频率、SpO_(2))。结果试验组患者停药后唤醒时间及气管导管拔管时间明显早于对照组,ICU停留时间明显少于对照组(P<0.05);试验组患者血流动力学指标(收缩压、舒张压、心率、呼吸频率)明显优于对照组(P<0.05),2组SpO_(2)之间的差异无统计学意义(P>0.05)。结论应用Narcotrend脑电/意识麻醉深度监测系统能够科学合理地评判镇静深度,使意识深度监测更精确、安全,利于体外循环术后患者镇静管理。Objective Purpose Application of Narcotrend EEG/conscious anesthesia depth monitoring system,Scientific and reasonable evaluation of sedation depth.Methods Random extraction 200 patients undergoing open heart surgery under cardiopulmonary bypass were randomly divided into experimental group and control group with 100 cases each in the chest ICU of a comprehensive tertiary first-class hospital in Nanchang from April 2018-April 2020,both groups of patients carried endotracheal tubes back to ICU to connect with multifunctional ventilator after operation and were continuously sedated by propofol according to the doctor's advice.During the use of sedatives,blood pressure,heart rate,SpO_(2) and respiratory rate were recorded every 30 minutes.The Narcotrend EEG/conscious anesthesia depth monitoring system was used to monitor the anesthesia depth in the experimental group,and the Ramsay sedation depth scoring system was used in the control group.the wake-up time,extubation time,ICU stay time,hemodynamic indexes(blood pressure,diastolic pressure,heart rate,respiratory rate and SpO_(2))of the two groups were compared.Results The wake-up time and tracheal extubation time of patients in the experimental group were significantly earlier than those in the control group,and the ICU stay time was significantly less than that in the control group.The hemodynamic indexes(blood pressure,diastolic pressure,heart rate and respiratory rate)of the experimental group were significantly better than those of the control group,and there was no significant difference in SpO_(2) between the two groups.Conclusion The application of Narcotrend EEG/consciousness anesthesia depth monitoring system can scientifically and reasonably judge the sedation depth,make the consciousness depth monitoring more accurate and safe,and facilitate the sedation management of patients after cardiopulmonary bypass.
