摘  要:目的观察脂肪来源干细胞治疗慢性创面的疗效,并探讨其对患者基质金属蛋白酶-2 (MMP-2)、基质金属蛋白酶-9 (MMP-9)表达水平的影响。方法收集2019年3月至2020年9月在阳江市人民医院烧伤整形外科接受治疗的82例慢性创面患者,依据随机数表法分为研究组和对照组,每组41例。研究组在常规综合疗法基础上联合脂肪来源干细胞治疗,对照组予以常规综合疗法。比较两组患者治疗结束后14 d的临床疗效,治疗结束后7 d及14 d的创面愈合率,治疗前及治疗结束后7 d时的MMP-2、MMP-9水平、创面量化积分和视觉模拟评分(VAS)。结果研究组患者的治疗总有效为率为92.68%,明显高于对照组的73.17%,差异有统计学意义(P<0.05);研究组患者治疗结束后7 d、14 d的创面愈合率分别为(9.86±1.12)%、(57.12±6.81)%,明显高于对照组的(6.53±0.85)%、(43.98±5.44)%,差异均有统计学意义(P<0.05);治疗结束后7 d,研究组患者的MMP-2、MMP-9分别为(1.43±0.18)μg/L、(3.28±0.42)μg/L,明显低于对照组的(1.89±0.23)μg/L、(4.06±0.55)μg/L,差异均有统计学意义(P<0.05);治疗结束后7 d,研究组患者的创面量化积分、VAS评分分别为(5.59±0.65)分、(1.12±0.14)分,明显低于对照组的(6.94±0.83)分、(1.83±0.25)分,差异均有统计学意义(P<0.05)。结论脂肪来源干细胞治疗慢性创面可降低患者的MMP-2、MMP-9水平,加速创面愈合,临床治疗效果显著。Objective To observe the therapeutic effect of adipose-derived stem cells on chronic wounds, and to explore its influence on the expression levels of matrix metalloproteinase-2(MMP-2) and matrix metalloproteinase-9(MMP-9) in patients. Methods A total of 82 patients with chronic wound who received treatment in the Department of Burn and Plastic Surgery, Yangjiang People’s Hospital from March 2019 to September 2020 were collected. They were divided into research group and control group according to random number table method, with 41 patients in each group.The research group received adipose-derived stem cell therapy on the basis of conventional combined therapy, while the control group received conventional combined therapy. The clinical efficacy at 14 days after completion of treatment,wound healing rate at 7 d and 14 d after completion of treatment, MMP-2 and MMP-9 levels, wound quantization score,and Visual Analog Scale(VAS) before and 7 d after completion of treatment were compared between the two groups.Results The total effective rate in the research group was 92.68%, which was significantly higher than 73.17% in the control group(P<0.05).
