射血分数保留的心力衰竭患者血清TSP-2、γ-GGT水平与心功能及心血管事件的关系

摘　　要：目的探讨射血分数保留的心力衰竭(HFpEF)患者血清血小板吸附蛋白2 (TSP-2)、γ-谷氨酰转移酶(γ-GGT)水平与心功能及心血管事件的关系。方法选择2017年1月至2019年1月延安大学咸阳医院心内科收治的80例HFpEF患者(HFpEF组)和50例门诊体检的健康志愿者(对照组)作为研究对象。采用酶联免疫吸附试验检测两组受检者的血清TSP-2、γ-GGT和B型利钠肽原(BNP)水平,超声心动图和三维斑点追踪技术检测两组受检者的左室射血分数(LVEF)、左心室整体应变(LVGS)、左心室纵向峰值应变(LVGLS)、圆周峰值应变(LVGCS)、径向峰值应变(LVGRS);采用Pearson相关性分析TSP-2、γ-GGT与患者血清BNP水平、LVEF、LVGS、LVGLS、LVGCS、LVGRS的相关性。以HFpEF组血清TSP-2、γ-GGT水平的均值为界,将患者分为高TSP-2水平组、低TSP-2水平组、高γ-GGT水平组、低γ-GGT水平组,Kaplan-Meier生存曲线分析TSP-2、γ-GGT水平与HFpEF患者心血管事件的关系。结果 HFpEF组患者的血清TSP-2、γ-GGT、BNP水平分别为(23.09±5.13) ng/mL、(36.65±6.12) U/L、(256.32±71.46) pg/mL,明显高于对照组的(14.15±2.21) ng/mL、(26.32±3.06) U/L、(22.25±12.65) pg/mL,差异均有统计学意义(P<0.05);HFpEF组患者的LVGS、LVGLS、LVGCS、LVGRS分别为(30.21±4.19)%、(16.34±3.26)%、(24.35±4.59)%、(39.23±5.46)%,明显低于对照组的(36.24±6.12)%、(22.34±2.46)%、(27.46±3.46)%、(44.25±6.15)%,差异均有统计学意义(P<0.05);相关性分析结果显示,TSP-2、γ-GGT与HFpEF患者血清BNP水平均呈正相关(r=0.563、0.505,P<0.05),与LVGS、LVGLS呈负相关(r=-0.473、-0.482;-0.591、-0.564,P<0.05);Kaplan-Meier生存曲线分析结果显示,高TSP-2组和高γ-GGT组患者的心血管事件发生率分别为31.37%、32.65%,明显高于低TSP-2组、低γ-GGT组患者的6.90%、6.45%,差异均有统计学意义(Log Rank2=4.857、5.618,P<0.05)。结论 HFpEF患者血清TSP-2、γ-GGT水平明显升高,高水平TSP-2、γ-GGT与HFpEF患�Objective To investigate the relationship between serum Thrombospondin-2(TSP-2) and γ-glutamine transferase(γ-GGT) levels and cardiac function and cardiovascular events in patients heart failure with preserved ejection fraction(HFpEF). Methods A total of 80 HFpEF patients, who admitted to Department of Internal Medicine-Cardiovascular, Xianyang Hospital of Yan’an University from January 2017 to January 2019, were selected as the HFpEF group, and 50 healthy volunteers who underwent outpatient physical examination were selected as the control group. The serum levels of TSP-2, γ-GGT and B-type natriuretic peptide(BNP) were detected by enzyme-linked immunosorbent assay, and left ventricular ejection fraction(LVEF), left ventricular global strain(LVGS), left ventricular global longitudinal strain(LVGLS), left ventricular global circumferential strain(LVGCS) and left ventricular global radial strain(LVGRS) were detected by echocardiography and three-dimensional(3 D) spot tracking. The relationship between TSP-2, γ-GGT and HFpEF patients serum BNP level, LVEF, LVGS, LVGLS, LVGCS and LVGRS were analyzed by Pearson correlation analysis. The mean values of serum TSP-2 and γ-GGT in HFpEF group were used as the boundary,and patients were divided into high TSP-2 level group, low TSP-2 level group, high γ-GGT level group and lowγ-GGT level group. Kaplan Meier survival curve was used to analyze the relationship between TSP-2, γ-GGT level and cardiovascular events in HFpEF patients. Results The serum levels of TSP-2, γ-GGT and BNP in HFpEF group were(23.09±5.13) ng/mL,(36.65±6.12) U/L,(256.32±71.46) pg/mL, respectively, which were significantly higher than corresponding(14.15±2.21) ng/mL,(26.32±3.06) U/L,(22.25±12.65) pg/mL in the control group(all P<0.05).
