摘 要:目的探讨远程心电监测系统在阵发性房颤监测中的应用价值。方法选取2019年1月至2020年9月该院收治的90例阵发性房颤患者为研究对象,采用随机数字表法分成研究组与对照组,各45例。研究组应用远程心电监测系统进行检查,对照组应用常规心电图进行检查。记录并比较两组心率、PR间期、QRS时限、QT间期、QRS振幅、Ⅰ导联T波振幅。比较有症状阵发性房颤患者与无症状阵发性房颤患者合并窦性心律的情况。比较两组阵发性房颤复发检出率、无症状阵发性房颤检出率。结果研究组Ⅰ导联T波振幅较对照组小,差异有统计学意义(P<0.05)。90例患者中共检出有症状阵发性房颤患者68例,无症状阵发性房颤患者22例,有症状阵发性房颤患者中窦性心律7例(10.3%),无症状阵发性房颤患者中窦性心律4例(18.2%),差异有统计学意义(P<0.05)。研究组阵发性房颤复发检出率、无症状阵发性房颤检出率均高于对照组,差异有统计学意义(P<0.05)。结论与常规心电图检查相比,远程心电监测系统的实时监测功能可早期诊断无症状阵发性房颤,早期评估复发情况,从而使临床能及时采取有效的治疗措施,防治疾病的进一步发展。Objective To explore the application value of remote ECG monitoring system in monitoring paroxysmal atrial fibrillation.Methods A total of 90 patients with paroxysmal atrial fibrillation admitted to the hospital from January 2019 to September 2020 were selected as the research objects,and they were divided into the study group and the control group using a random number table method,with 45 cases in each group.The study group was examined by remote ECG monitoring system,and the control group was examined by routine ECG.The heart rate,PR interval,QRS duration,QT interval,QRS amplitude,and leadⅠT wave amplitude were recorded and compared between the two groups.Compared the situation of sinus rhythm in patients with symptomatic paroxysmal atrial fibrillation and asymptomatic paroxysmal atrial fibrillation.Compared the recurrence detection rate of paroxysmal atrial fibrillation and the detection rate of asymptomatic paroxysmal atrial fibrillation between the two groups.Results The leadⅠT wave amplitude in the study group was smaller than that in the control group,and the difference was statistically significant(P<0.05).A total of 68 patients with symptomatic paroxysmal atrial fibrillation and 22 patients with asymptomatic paroxysmal atrial fibrillation were detected in 90 patients,7 patients with symptomatic paroxysmal atrial fibrillation had sinus rhythm(10.3%),and 4 patients with asymptomatic paroxysmal atrial fibrillation had sinus rhythm(18.2%),the difference was statistically significant(P<0.05).The recurrence detection rate of paroxysmal atrial fibrillation and the detection of asymptomatic paroxysmal atrial fibrillation in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with routine ECG examinations,the real-time monitoring function of the remote ECG monitoring system could diagnose asymptomatic paroxysmal atrial fibrillation early,and evaluate the recurrence early,so that effective clinical measures can be taken in
