切除袋形缝合和切口开放搔刮术治疗骶尾部藏毛窦的效果比较

摘　　要：为观察切除袋形缝合和切口开放搔刮术在骶尾部藏毛窦中的效果,将46例骶尾部藏毛窦患者随机分为A组和B组,每组23例。A组患者采用切除袋形缝合术治疗,B组采用切口开放搔刮术治疗,比较2组患者手术指标、住院时间及术后创口疼痛程度,观察2组手术疗效及术后并发症发生情况。结果显示,2组患者住院时间比较差异无统计学意义(P>0.05),A组患者手术时间长于B组,创口愈合时间短于B组(P<0.05);术后24h2组患者疼痛评分比较差异无统计学意义(P>0.05),术后48h2组患者疼痛评分均降低(P<0.05),B组疼痛评分低于A组(P<0.05);2组患者均治愈,2组患者术后并发症发生率比较差异无统计学意义(P>0.05)。结果表明,切除袋形缝合和切口开放搔刮术在骶尾部藏毛窦治疗中效果显著,切除袋形缝合创口愈合快,切口开放搔刮手术时间短,术后疼痛轻,2种手术方式均有较高的临床应用价值,临床应根据患者实际病情选用合适的手术方式。This study was to observe and compare the effect of resection & marsupialization(R & M procedure)and incision open &scratch shave(IOSS procedure)in the treatment of sacrococygeal pilonidal sinus(SPS),randomly divided 46 SPS patients into group A(23 cases,adopted the R & M procedure)and group B(23 cases,IOSS procedure);then,compared both groups’ operative norms,hospitalization time,postoperative pain intensity,operative effect,as well as postoperative complication incidence status.As results,in hospitalization time,in the score ratings on pain 24 hrs after surgery,and in postoperatie complication incidence,there all were no statistical difference between the two groups(P >0.05,all),the 46 patients were cured;and in operating time group A was longer than groups B(P<0.05),in healing time group A was shorter than group B(P<0.05);48 hrs after surgery both groups’ pain ratings were declined(P<0.05),but in group B,the ratings were lower than that in group A(P<0.05).Results show that both procedures for SPS all have significant effect,and high curative rate;merely,R & M procedure’s wound healing faster,IOSS procedure’s operating time shorter and postoperative pain slighter,thus,it should be according to patients’ concrete condition to opt one of both.
