摘　　要：目的分析河北省2010-2019年艾滋病(AIDS)流行特征和变化趋势,为制订预防控制措施提供科学依据。方法收集2010年1月1日-2019年12月31日河北省新报告的HIV/AIDS病例个案信息,用EXCEL2010建立数据库,对报告HIV/AIDS患者的流行概况、地区分布、传播途径及年龄、性别、婚姻状况、文化程度等人口学特征进行统计分析。结果 2010-2019年,河北省新报告HIV/AIDS 14 819例,报告发病率从2010年的0.53/10万上升到2019年的3.30/10万。报告死亡病例1 854例,病死率由2010年的6.24%下降至2019年的1.89%。报告病例前5的市分别为石家庄、保定、唐山、沧州、廊坊。新报告病例数超过100例的县区有43个。男女性别比为9.1∶1。发现时平均年龄为(37.27±13.60)岁。20~49岁报告病例占76.2%。同性传播占64.2%,异性传播占32.7%。异性传播男性病例中婚内传播占3.5%,异性传播女性病例中婚内传播占42.1%。患者文化程度以初中为主,占38.9%,婚姻状况以已婚为主,占47.2%。报告病例主要来自医疗机构占45.1%。结论河北省AIDS病例呈逐年增加趋势,同性传播为主要传播途径,应重点关注50岁以上中老年人口AIDS问题和男性病例家庭内传播。进一步加强AIDS健康宣教和检测力度,有效地遏制疫情蔓延。Objective To analyze the epidemiological characteristics of and trends in AIDS in Hebei Province in order to provide a scientific basis for formulating prevention and control measures. Methods Information on newly reported HIV/AIDS cases in Hebei Province from 2010 to 2019 was collected. A database was created using Excel 2010, and the data were analyzed using SPSS23.0. The demographic characteristics of patients with HIV/AIDS, such as the prevalence, regional distribution, route of transmission, age, sex, marital status, and level of education, were statistically analyzed. Results There were 14,819 new cases of HIV/AIDS reported in Hebei Province from 2010 to 2019, and the reported incidence increased from 0.53 per 100,000 in 2010 to 3.30 per 100,000 in 2019. In the province, 1,854 deaths were reported, and the case fatality rate decreased from 6.24% in 2010 to 1.89% in 2019. The top 5 cities with reported cases were Shijiazhuang, Baoding, Tangshan, Cangzhou, and Langfang. There were 43 counties and districts where the number of newly reported cases exceeded 100. The sex ratio of male and female patients was 9.1:1. The average age at the time of discovery was 37.27±13.60 years. The average age of new patients with HIV/AIDS increased from 33.86 years in 2010 to 38.91 years in 2019. Patients ages 20 to 49 accounted for 76.2% of reported cases. The proportion of patients age 50 or older increased from 10.3% in 2010 to 24.6% in 2019. Homosexual transmission accounted for 64.2% of cases, and heterosexual transmission accounted for 32.7%. Among males involved in heterosexual transmission, 3.5% transmitted HIV/AIDS to their spouse. Among females involved in heterosexual transmission, 42.1% transmitted HIV/AIDS to their spouse. Most patients(38.9%) had a middle school level of education. Most patients(47.2%) were married. The reported cases were mainly from medical facilities(45.1%). Blood donor testing identified 6.1% of the cases. Conclusion The number of AIDS cases in Hebei Province has increased yearly. Homosexual
