巨大破裂型腰椎间盘突出症重吸收55例分析

摘　　要：目的:回顾性分析55例发生接近完全重吸收的巨大破裂型腰椎间盘突出症(Lumbar Disc Herniation,LDH)患者的临床及影像学资料,寻找其共性。方法:以2008年至2019年期间,资料完整、重吸收率大于80%的55例巨大破裂型腰椎间盘突出症患者为研究对象,对患者的年龄分布、重吸收时间、MRI特点、日本骨科协会(Japanese Orthopaedic Association,JOA)评分等进行分析评定。结果:患者年龄多集中在30~49岁,占患者的69.90%;吸收时间多为6个月以上,占患者的76.36%;MRI分布类型中Komori 3型占72.73%;治疗前JOA评分为(12.18±3.42)分,治疗后为(26.65±1.57)分,两者比较差异有统计学意义(P<0.05)。结论:发生接近完全重吸收的巨大破裂型腰椎间盘突出症患者,年龄多集中在30~49岁,吸收的时间多为6个月以上,Komori 3型最容易发生重吸收。对于巨大、破裂型腰椎间盘突出症,非手术治疗是一种可以选择的治疗方式。Objective:To retrospectively analyze the clinical and imaging data of 55patients with massive ruptured lumbar disc herniation(LDH)that was close to complete resorption,and find the commonalities which were of significance to the clinical treatment.Methods:A retrospective study was performed on 55massive ruptured LDH patients who had complete data and over 80%resorption rate from 2008to 2019.The age distribution,resorption time,MRI characteristics,and JOA score of the patients were analyzed and evaluated.Results:The patients were mostly aged 30to 49years old,accounting for 69.90%of the patients.The absorption time was more than 6months,accounting for 76.36%.Komori 3accounted for 72.73%of the MRI distribution types.JOA score before and after treatment were(12.18±3.42)points and(26.65±1.57)points,and the differences were statistically significant(P<0.05).Conclusion:Patients with massive ruptured LDH who have nearly complete reabsorption are mostly between 30and 49years old,and the absorption time is more than 6 months.Komori 3is the most prone to reabsorption.Non-surgical treatment is an alternative treatment for massive and ruptured LDH.
