摘  要:目的探讨超声检查早孕期胎儿中枢神经系统(CNS)畸形的临床价值。方法选取我院1646例早孕期的孕妇进行超声检查,统计头胸部正中矢状断面、脊柱长轴断面、侧脑室水平横断面、小脑与颅后窝池断面显示情况。结果孕11~11+6周孕妇头胸部正中矢状断面、脊柱长轴断面、侧脑室水平横断面及小脑与颅后窝池断面的超声显示率分别为97.47%、98.31%、99.16%、98.73%,与孕12~12+6周(98.21%、98.88%、99.44%、98.44%)及孕13~13+6周(98.05%、99.03%、98.25%、98.64%)比较差异无统计学意义(P>0.05);早孕期超声检查的灵敏度为90.00%,特异性为100%,阳性预测值为100%,阴性预测值为99.94%。结论超声检查早孕期胎儿CNS畸形具有较高的显示率,为临床治疗提供依据。Objective To investigate the value of ultrasound in the diagnosis of fetal CNS malformation in early pregnancy.Methods 1646 cases of pregnant women in early pregnancy(11~13+6 weeks of gestation)were selected from September 2018 to September 2020.All pregnant women received ultrasound examination.The ultrasound findings of different sections(including head chest median sagittal section,spine long axis section,lateral ventricle horizontal section,cerebellum and posterior fossa cistern section)were analyzed.The ultrasound screening results of late pregnancy and the situation of CNS of pregnant women after induction of labor or birth were taken as the gold standard for diagnosis,and the sensitivity of ultrasound examination was calculated.Results There was no significant difference(P>0.05)between 12~12+6 weeks of gestation(98.21%,98.88%,99.44%,98.44%)and 13~13+6 weeks of gestation(98.05%,99.03%,98.25%,98.64%);The sensitivity,specificity,positive predictive value and negative predictive value of ultrasonography in early pregnancy were 90.00%(9/10),100.00%(1636/1636),100.00%(9/9)and 99.94%(1636/1637).Conclusion Early pregnancy(11~13+6 weeks of gestation)pregnant women using ultrasound has a high display rate;and the reasonable application of ultrasound in the diagnosis of fetal CNS malformation in early pregnancy has a significant effect,which can clearly diagnose the disease,with high clinical diagnosis rate,and can provide scientific basis for the follow-up reasonable treatment of CNS malformation fetus.
