摘 要:目的分析70 kV管电压联合大螺距扫描(Flash)技术在新生儿胸部CT的图像质量及辐射剂量,探讨其应用可行性。方法选取我院诊疗的新生儿60例,随机分为A、B两组,每组30例。A组利用70 kV联合Flash技术进行扫描,B组利用常规120 kV管电压进行扫描,利用统计学方法比较两组图像的主、客观图像质量及辐射剂量的差异。结果所有病例均成功完成检查,其中A、B两组图像主观质量评分分别为(3.6±0.5 vs 4.2±0.8,P=0.215)。客观图像质量评分主要指标方面:A组图像噪声高于B组(13.4±4.7 vs 8.8±4.3,P=0.149),差异无统计学意义。SNR B组优于A组(5.2±1.5 vs 8.5±5.7,P=0.258),差异无统计学意义。B组CNR优于A组(14.6±4.5 vs 23.2±13.9,P=0.226)。辐射剂量方面:两组剂量长度乘积(DLP)、CT容积剂量指数(CTI vol)、有效辐射剂量(ED)分别为:(6.8±3.1)mGy·cm vs(44.6±3.8)mGy·cm,P=0.000;(0.39±0.13)mGy vs(2.17±0.28)mGy,P=0.000;(0.26±0.12)mSv vs(1.73±0.15)mSv,P=0.000,A组显著低于B组,差异有统计学意义。结论70 kV管电压联合Flash技术的新生儿胸部CT图像质量满足诊断的同时可以显著降低辐射剂量,可以应用于临床诊断。Objective The aim of study is to explore the feasibility using flash and 70 kV chest in infant.Methods The study was approved by the institutional review board.The image data from 60 patients were divided into Group A and B.Group A received high-pitch 70 kV CT chest examinations and B underwent standard 120 kV chest CT imaging.All CT scans were performed on a third-generation dual-source CT unit.The objective image quality was evaluated using signal-to-noise(SNR)and contrast-to-noise ratios(CNR),noise.Two blinded and independent radiologists evaluated subjective image quality and diagnostic confidence using 5-point Likert scales.For an evaluation of the radiation dose,the CT dose index(CTDIvol),dose-length product(DLP),effective dose(ED)were analyzed in each group.Results The objective image parameters(SNR:5.2±1.5 vs 8.5±5.7,P=0.258;CNR:14.6±4.5 vs 23.2±13.9,P=0.226;Noise:13.4±4.7 vs 8.8±4.3,P=0.149)and the cumulative subjective image quality(3.6±0.5 vs 4.2±0.8,P=0.215)showed no significant differences between the two protocols.The mean dose parameters were significantly lower for high-pitch 70 kV CT examinations〔CTDIvol:(0.39±0.13)mGy;DLP:(6.8±3.1)mGy·cm;ED:(0.26±0.12)mSv〕;compared to standard 120 kV CT imaging〔CTDI vol:(2.17±0.28)mGy,P=0.000;DLP:(44.6±3.8)mGy·cm,P=0.000;ED:(1.73±0.15)mSv,P=0.000〕.Conclusion High-pitch 70 kV thoracic CT examinations in infant resulted in a significantly reduced radiation exposure compared to standard 120 kV CT acquisition without a decrease in image quality.
