摘 要:目的探讨不同体质量指数(BMI)对慢性心力衰竭患者运动耐量及静息心率的影响。方法对156例慢性心力衰竭患者进行分析,根据BMI数值分为正常组(55例)、超重组(49例)和肥胖组(52例)。每组患者均给予相同的标准方案抗心衰治疗3个月,分别记录治疗开始和治疗3个月后6 min步行试验距离和静息心率。利用随机样本t检验,对以上患者治疗前与治疗后的运动耐量、静息心率、BNP及hs-CRP进行分析。结果经过3个月治疗,肥胖组患者6 min步行试验的距离低于正常组患者[(240.71±29.05)m vs.(254.47±28.29)m,P<0.05],超重组患者静息心率[(73.47±5.36)次/min vs.(70.13±5.17)次/min]和肥胖组静息心率[(74.69±5.84)次/min vs.(70.13±5.17)次/min]均高于正常组(P<0.01),正常组患者运动耐量改善情况和静息心率控制情况均优于超重组和肥胖组(P<0.01);超重组和肥胖组患者BNP和hs-CRP水平均高于正常组患者(P<0.01),超重组与肥胖组间比较差异有统计学意义(P<0.05)。结论高BMI降低慢性心力衰竭患者的运动耐量,同时影响静息心率的控制,降低BNP和hs-CRP水平。Objective To investigate the effects of different body mass index(BMI)on exercise tolerance and resting heart rate in patients with chronic heart failure(CHR).Methods A total of 156 patients with CHR were enrolled in the study,who were divided into control group(n=55),overweight group(n=49)and obesity group(n=52)according to BMI.The patients in the three groups were treated by the same protocol for 3 months.The 6-minute walking distance,resting heart rate,and exercise tolerance,resting heart rate,BNP and hs-CRP levels before and after treatment were statistically analyzed and compared among the three groups.Results After 3-month treatment,the 6-minute walking distance in obesity group was significantly lower than that in control group(P<0.05),however,the resting heart rates in overweight group and obesity group were significantly higher than those in control group(P<0.05).Moreover the improvement status of exercise tolerance and control status of resting heart rate in control group were superior to those in overweight group and obesity group(P<0.05),and the levels of BNP and hs-CRP in overweight group and obesity group were significantly higher than those in control group(P<0.05).There were significant differences between overweight group and obesity group(P<0.05).Conclusion The high BMI will decrease the exercise tolerance of patients with CHR and affect the control of resting heart rate,and decrease the levels of BNP and hs-CRP.
