血清HO-1、VEGF水平与2型糖尿病患者视网膜病变的相关性

摘　　要：目的:探讨血清血红素氧合酶-1(HO-1)、血管内皮生长因子(VEGF)与2型糖尿病(T2DM)患者视网膜病变的相关性。方法:选取某院2020年2月~2021年3月收治的120例T2DM患者为研究对象,入院时检测患者血清HO-1、VEGF水平,采用眼底彩色机进行眼底照相,根据有无发生糖尿病视网膜病变(DR)分为发生组和未发生组,分析血清HO-1、VEGF水平与T2DM患者DR的相关性。结果:120例患者中48例发生DR占40.00%(48/120),72例未发生DR占60.00%(72/120);发生组血清HO-1水平低于未发生组,VEGF水平高于未发生组,差异有统计学意义(P<0.05);两组其他基线资料比较,差异无统计学意义(P>0.05);经Logistic回归分析结果显示,血清HO-1水平低表达、VEGF水平高表达与T2DM患者DR相关(P<0.05);绘制受试者工作曲线(ROC),血清HO-1、VEGF单项及联合预测T2DM患者发生DR的曲线下面积(AUC)分别为0.769、0.825、0.849,根据最佳截断值,HO-1+VEGF评估T2DM患者发生DR的AUC、灵敏度均高于单项指标检测,其诊断价值更高。结论:血清HO-1、VEGF表达与T2DM患者DR密切相关,血清HO-1水平低表达、VEGF水平高表达提示患者DR发生风险较高。Objective:To investigate the correlation between serum heme oxygenase-1(HO-1),vascular endothelial growth factor(VEGF)and retinopathy in patients with type 2 diabetes(T2DM).Methods:A total of 120 T2DM patients admitted to a hospital from February 2020 to March 2021 were selected as the research objects.The serum HO-1 and VEGF levels of the patients were detected on admission,and fundus photography was performed with a fundus color machine.According to the occurrence of diabetic retinopathy(DR),they were divided into occurrence group and non-occurrence group;The correlation between serum HO-1,VEGF levels and T2DM patients were analyzed.Results:48 of the 120 patients developed DR,accounting for 40.00%(48/120),and 72 cases did not develop DR,accounting for 60.00%(72/120).The serum HO-1 level of the occurrence group was lower than that of the non-occurrence group,the VEGF level was higher than that in the non-occurrence group,and the difference was statistically significant(P<0.05).Compared with other baseline data between the two groups,the difference was not statistically significant(P>0.05).Logistic regression analysis showed that low expression of serum HO-1 level and high expression of VEGF level were associated with DR in T2DM patients(P<0.05).The receiver operating curve(ROC)was drawn,and the area under the curve(AUC)of serum HO-1,VEGF single item and combined prediction of DR in T2DM patients were 0.769,0.825,and 0.849 respectively.According to the best cut-off value,the AUC and sensitivity of HO-1+VEGF in assessing the occurrence of DR in T2DM patients were higher than those of single index detection,and its diagnostic value was higher.Conclusion:The expression of serum HO-1 and VEGF is closely related to DR in patients with T2DM.The low expression of serum HO-1 and high expression of VEGF indicate a higher risk of DR in patients.
