慢性化脓性中耳炎患者咽鼓管功能及其影响因素探究

摘　　要：目的:探究慢性化脓性中耳炎患者咽鼓管功能及其影响因素。方法:研究对象为慢性化脓性中耳炎患者,共80例(126耳),时间为2017年4月~2020年3月,将其列为观察组,再选取30例(42耳)正常人为对照组,对两组患者咽鼓管功能及其影响因素进行对比。结果:观察组ETS评分低于对照组(P<0.05);单因素分析后发现,不同年龄、病程的患者其ETS评分无统计学差异(P>0.05),但是无乳突鼓窦炎性病变组ETS评分高于有乳突鼓窦炎性病变组、无中耳肉芽组ETS评分高于有中耳肉芽组(P<0.05);Logistic多因素分析结果显示,慢性化脓性中耳炎患者有无中耳肉芽与其咽鼓管功能有关(P<0.05),但是患者的年龄、病程、有无乳突鼓窦炎性病变与其咽鼓管功能无关(P>0.05)。结论:慢性化脓性中耳炎患者其咽鼓管功能比较差,尤其是经CT检查发现有乳突鼓窦炎性病变及术中探查发现有中耳肉芽的患者,其存在咽鼓管障碍的几率更高。Objective:To explore the eustachian tube function and its influencing factors in patients with chronic suppurative otitis media.Methods:A total of 80 patients with chronic suppurative otitis media(126 ears)treated in a hospital from April 2017 to March 2020 were slected and they were listed as the observation group.30 cases(42 ears)of normal people were selected as the control group.The function of oropharachian tube and its influencing factors were compared between the two groups.Results:The ETS score of the observation group was lower than that of the control group(P<0.05).After univariate analysis,it was found that the ETS scores of patients with different ages and course of disease had no statistical difference(P>0.05),but the ETS score of the group without mastoid tympanic sinus inflammatory disease was higher than that of the group with mastoid tympanic sinus inflammatory disease and the ETS score of group without middle ear granulation was higher than that of the group with middle ear granulation(P<0.05).The logistic multivariate analysis showed that the ETS score of the group without mastoid tympanic sinus inflammatory disease was higher than that of the group with middle ear granulation(P<0.05).The results showed that the middle ear granulation was related to eustachian tube function in patients with chronic suppurative otitis media(P<0.05),but the age,course of disease,mastoid tympanic sinus inflammatory lesions were not related to eustachian tube function(P>0.05).Conclusion:The eustachian tube function of patients with chronic suppurative otitis media is poor,especially the patients with mastoid tympanic sinus inflammatory lesions found by CT examination and middle ear granulation found by intraoperative exploration have a higher probability of eustachian tube dysfunction.
