小切口外痔剥离加内痔缝扎术治疗环状混合痔的临床效果

摘　　要：目的:探讨小切口外痔剥离加内痔缝扎术治疗环状混合痔的临床效果。方法:选择2019年3月~2021年3月于某院治疗的84例环状混合痔患者,按随机数字表法将其分为观察组和对照组各42例,观察组采用小切口外痔剥离加内痔缝扎术治疗,对照组采用传统外剥内扎术治疗,观察至术后2周,比较两组临床疗效、围术期指标、肛门功能、生活质量及并发症。结果:两组治疗总有效率(RR)对比,无统计学差异(P>0.05);观察组术中出血量为(8.65±2.06)ml,少于对照组;手术时间、排便时间、创面愈合时间为(14.29±3.12)min、(2.47±1.05)d、(8.56±1.72)d,均短于对照组;视觉模拟评分法(VAS)评分为(3.64±0.75)分,低于对照组;术后Wexner评分为(2.39±0.97)分,低于对照组;健康调查简表(SF-36)评分为(80.07±7.12)分,高于对照组;并发症总发生率(4.76%)低于对照组,有统计学差异(P<0.05)。结论:环状混合痔患者采用小切口外痔剥离加内痔缝扎术治疗优于传统外剥内扎术,手术损伤小、术后恢复快及并发症少,能够加快肛门功能恢复,提高生活质量,可作为治疗环状混合痔的优选术式。Objective:To investigate the clinical effect of small incision external hemorrhoids dissection and internal hemorrhoid sutures in the treatment of circular mixed hemorrhoids.Methods:A total of 84 patients with circular mixed hemorrhoids treated in a hospital from March 2019 to March 2021 were selected and divided into two groups according to the random number table method,with 42 cases in each group.The observation group was treated with small incision external hemorrhoids dissection and internal hemorrhoids suture ligation,and the control group was treated with traditional external dissection and internal ligation,and the observation was performed until 2 weeks after the operation.The clinical efficacy,perioperative indicators,anal function,quality of life and complications were compared between the two groups.Results:The total effective rate(RR)of the two groups was compared,and there was no statistical difference(P>0.05).The intraoperative blood loss of the observation group was(8.65±2.06)ml,which was less than that of the control group,the operation time,defecation time,and wound healing time were(14.29±3.12)min,(2.47±1.05)d,(8.56±1.72)d,which were shorter than those of the control group.The visual analogue scoring(VAS)score was(3.64±0.75)points,which was lower than that of the control group.The post Wexner score was(2.39±0.97)points,which was lower than that of the control group,and the health survey summary form(SF-36)score was(80.07±7.12)points,which was higher than that of the control group,and the total incidence of complications in the observation group(4.76%)was lower than that in the control group,there was a statistical difference(P<0.05).Conclusion:The treatment of circular mixed hemorrhoids with small incision external hemorrhoid dissection and internal hemorrhoid suture ligation is better than traditional external hemorrhoid dissection and internal ligation,with less surgical damage,faster postoperative recovery and fewer complications.It can speed up the recovery of anal function and impro
