摘 要:目的分析首发Tourette综合征患者阿立哌唑治疗前后脑功能性磁共振成像(fMRI)变化。方法选取2018年1-12月收治的30例首发Tourette综合征患者为观察组,30例健康受试者为对照组。所有受试者治疗前后均进行静息态fMRI扫描,在0.01~0.10 Hz频段内计算脑比率低频振幅(fALFF)值,比较两组治疗前后fALFF值,观察组治疗前后fALFF值,观察组治疗前后耶鲁综合抽动严重程度量表(YGTSS)评分。结果治疗前,观察组患者左侧梭状回、右侧壳核、右侧苍白球、边缘叶及海马回fALFF值均强于对照组,左侧额下回三角区、左侧额上回、左侧枕中回、左侧楔前叶、右侧额中回、双侧罗兰迪克岛盖及顶下小叶fALFF值均弱于对照组,差异均有统计学意义(均P<0.05)。治疗后,观察组左侧梭状回、右侧壳核、右侧苍白球、边缘叶及海马回fALFF值均强于对照组,左侧额下回三角区、左侧额上回、左侧枕中回、左侧楔前叶、右侧额中回、两侧罗兰迪克岛盖及顶下小叶fALFF值均弱于对照组(均P<0.05)。观察组治疗后左侧梭状回、右侧壳核、右侧苍白球、边缘叶及海马回fALFF值均弱于治疗前,左侧额下回三角区、左侧额上回、左侧枕中回、左侧楔前叶、右侧额中回、两侧罗兰迪克岛盖、顶下小叶fALFF值均强于治疗前(均P<0.05)。结论静息态fMRI扫描技术对于判断Tourette综合征患者严重程度和研究Tourette综合征的发病机理有重要意义,且药物治疗在一定程度上可纠正这些异常。Objective To analyze the changes of brain functional magnetic resonance imaging(fMRI)before and after aripiprazole treatment in patients with first-episode Tourette syndrome.Methods From January to December in 2018,30 patients with first-episode Tourette syndrome and 30 healthy children were selected as observation group and control group,respectively.Before and after treatment,resting state fMRI scan was performed,brain ratio low-frequency amplitude(fALFF)levels were calculated within the frequency range of 0.01-0.10 Hz.fALFF levels before and after treatment in the two groups,fALFF levels before and after treatment in observation group,Yale comprehensive tic severity scale(YGTSS)scores were compared.Results Before treatment,fALFF levels of left fusiform gyrus,right putamen,right globus pallidus,limbic lobe,and parahippocampal gyrus in observation group were stronger than those in control group,fALFF levels of left gyrus frontalis inferior trigonum,left gyrus frontalis superior,left middle occipital gyrus,left precuneus,right middle frontal gyrus,bilateral rolandic operculum,and inferior parietal lobule were weaker than those in control group,there were statistically significant differences(P<0.05).After treatment,fALFF levels of left fusiform gyrus,right putamen,right globus pallidus,limbic lobe,and parahippocampal gyrus in observation group were stronger than those in control group,fALFF levels of left gyrus frontalis inferior trigonum,left gyrus frontalis superior,left middle occipital gyrus,left precuneus,right middle frontal gyrus,bilateral rolandic operculum,and inferior parietal lobule were weaker than those in control group,there were statistically significant differences(P<0.05).In observation group,fALFF levels of left fusiform gyrus,right putamen,right globus pallidus,limbic lobe,and parahippocampal gyrus after treatment were weaker than those before treatment,fALFF levels of left gyrus frontalis inferior trigonum,left gyrus frontalis superior,left middle occipital gyrus,left precuneus,right middle fr
