布鲁菌骨关节病患者外周血TNF-α、IL-6、IL-17的变化及意义

摘　　要：目的探讨布鲁菌骨关节病患者外周血清中白细胞介素6(interleukin 6,IL-6)、白细胞介素17(interleukin 17,IL-17)和肿瘤坏死因子α(tumor necrosis factor alpha,TNF-α)的表达水平及临床意义。方法选取2018年1月-2020年1月内蒙古医科大学附属医院收治的80例布鲁菌病未合并骨关节病患者(对照组)和80例布鲁菌病合并骨关节病患者(骨关节病组)。酶联免疫吸附实验检测2组血清TNF-α、IL-6、IL-17水平,多因素分析采用Logistic回归模型,受试者工作特征曲线分析血液指标预测治愈率的价值并计算曲线下面积(AUC),对2组结果进行比较。结果骨关节病组血清TNF-α、IL-6、IL-17水平均高于对照组(P<0.05)。多因素分析结果显示,降钙素原(PCT)、C反应蛋白(CRP)、血沉、IL-6、TNF-α和IL-17是布鲁菌骨关节病发生的独立影响因素(P<0.05)。IL-6、TNF-α和IL-17诊断布鲁菌骨关节病的AUC分别为0.842、0.816、0.888,均高于PCT(0.680)、CRP(0.777)、血沉(0.712)。联合上述6个指标可将AUC提高至0.931。结论布鲁菌骨关节病患者血清TNF-α、IL-6、IL-17表达水平升高,对早期诊断布鲁菌骨关节病有一定价值。Objective To investigate the expression levels and clinical significance of interleukin 6(IL-6),IL-17 and tumor necrosis factor alpha(TNF-α)in peripheral serum of Brucella osteoarthropathy patients.Methods From January 2018 to January 2020,80 patients with brucellosis without osteoarthropathy(control group)and 80 patients with brucellosis with osteoarthropathy(osteoarthropathy group)were studied.Enzyme-linked immunosorbent assay was used to detect serum TNF-α,IL-6 and IL-17 levels.Multivariate analysis was performed using Logistic regression model;receiver operating characteristic curve was to analyze the value of blood indicators to predict cure rate,and to calculate the area under the curve(AUC).Results The levels of serum TNF-α,IL-6 and IL-17 in the osteoarthropathy group were higher than those in the control group(P<0.05).Multivariate analysis results showed that procalcitonin(PCT),C-reactive protein(CRP),erythrocyte sedimentation rate,IL-6,TNF-αand IL-17 are independent factors influencing the occurrence of Brucella osteoarthropathy(P<0.05).The AUC of IL-6,TNF-αand IL-17 in the diagnosis of Brucella osteoarthropathy were 0.842,0.816 and 0.888 respectively,which were higher than PCT(0.680),CRP(0.777)and ESR(0.712).Combining the above six indicators can increase the AUC up to 0.931.Conclusion Serum TNF-α,IL-6 and IL-17 expression levels in patients with Brucella osteoarthropathy have a certain value for early diagnosis of Brucella osteoarthropathy.
