摘  要:目的分析新疆生产建设兵团(简称兵团)第五师手足口病实验室检测结果,了解手足口病流行病学及病原学特征。方法2011—2019年采集兵团第五师疑似手足口病患者粪便标本133份,应用荧光RT-PCR方法检测肠道病毒(HEV)、肠道病毒71型(EV 71)、柯萨奇病毒A组16型(Cox A16)、柯萨奇病毒A组6型(Cox A6)和柯萨奇病毒A组10型(Cox A10)的特异性核酸,选取部分阳性标本扩增VP 1序列、测序并分析序列。结果133份标本,检出阳性123份,临床标本实验室检测总阳性率92.48%;患者男性多于女性,1~5岁病例最多(109例、占81.95%);4月开始发病并呈上升态势至6月后逐渐下降,5—8月为发病高峰期;核酸检测肠道病毒病原分型以Cox A16(46.34%)、EV 71(23.58%)和Cox A6(17.89%)为主,其他未分型占12.19%;病毒VP 1基因序列分析,病例标本中Cox A16均属B1b亚型、Cox A6均属D2亚型,与国内近年某些地区流行株有较近亲缘关系。结论兵团第五师手足口病主要以EV 71,Cox A16和Cox A6感染为主,存在感染人群年龄组和季节性差异;应根据当地手足口病流行病学及病原学特征制定政策,强化当地手足口病防控措施。Objective To understand the epidemiological and etiological characteristics by analyzing laboratory test results of hand-foot-mouth disease(HFMD)in The Fifth Division of Xinjiang Production and Construction Corps from 2011 to 2019.Methods From 2011 to 2019,133 feces samples of suspected patients with HFMD were collected to detect the specific nucleic acid of enterovirus 71(EV 71),Coxsackie virus A16(Cox A16),Coxsackie virus A6(Cox A6)and Coxsackie virus A10(Cox A10)with the real-time fluorescent RT-PCR.Part of the positive samples were selected to make VP 1 sequence amplification,genome sequencing and sequence analysis.Results Out of the 133 samples,123 were tested positive,with the total positive rate of 92.48%at the clinical samples laboratory,and the patients was more in the male than in the females.There were 109 patients at most in the age group of 1-5-year,accounting for 81.95%.The onset of the disease started from April with raising trend,then to June decreased gradually,with the peak of onset from May to August.The enterovirus pathogen typing was mainly Cox A16(46.34%),EV 71(23.58%)and Cox A6(17.89%),and others without typing accounted for 12.19%.VP 1 sequencing analysis displayed Cox A16 strains of all subtype B1 b,Cox A6 strains of subtype D2,and there were closely genetic relationships with the epidemic strains in some areas of China in recent years.Conclusions HFMDs are mainly infected with EV71,Cox A16 and Cox A6 in The Fifth Division of Xinjiang Production and Construction Corps,and the differences among age groups and seasons exist in the infected population.The policies should be formulated base on the local epidemiological and etiological characteristics of HFMD to strengthen the prevention and control of HFMD.
